Study On Evaluation Index System Of Payment Reform Based On Disease Diagnosis Related Groups(DRGs)

ObjectiveThis paper aims to put forward the evaluation framework of China's DRGs payment system reform.And to provide a scientific reference for the construction and evaluation of DRGs payment system in China.Methods1.Through the literature review method,collect domestic and foreign literature research on DRGs payment system reform evaluation,and summarize,sort out,analyze,screen and refine key evaluation indicators,and systematically sort out domestic and foreign domestic and international DRGs payment system reform effect evaluation framework.And initially form the DRGs payment system reform evaluation system.2.Through the Delphi method,summarize and analyze the importance of indicators in the index pool of the initial proposed DRG payment system reform evaluation,and initially construct the DRGs payment system reform evaluation framework.3.Apply mathematical statistics method to investigate and analyze the consulting experts,analyze the basic situation of the expert composition,and the relationship between the expert grouping characteristics and the expert score.4.Through the AHP method to conduct expert consultation on the initial evaluation framework for constructing the DRG payment system reform.According to the expert score,compare the relative importance between the indicators,determine the weight of the indicators,and construct a set of stratified evaluation index system.Results1.Review the research progress of the evaluation of DRGs payment reform at home and abroad.2.The theoretical model of the DRGs payment system reform evaluation framework with the four basic elements of hospital goal dimension including government goal dimension,patient goal dimension and medical goal dimension as the main evaluation dimension has been established.3.Construct an evaluation index system for the reform of the payment system of DRGs.According to the principle of index selection,the DRGs evaluation index pool was selected.Through two rounds of expert consultation,the DRGs evaluation index system including four first-level indicators,the second-level indicators,and the third-level indicators was determined,and the weights of the indicators at each level were determined by the stratified analysis method.Conclusions1.Based on the literature research,the theoretical model of the evaluation framework of the four-dimensional DRGs payment method for hospitals,government,patients and medicine is relatively comprehensive,which provides a reference for comprehensive evaluation of the reform effect of DRGs payment methods in China.2.The DRGs payment method reform evaluation index system based on Delphi method and AHP is more accurate and credible.3.At present,the evaluation research on the reform of payment methods for DRGs in China is still in the exploratory stage.This study has laid a foundation for the establishment and improvement of DRGs related evaluation research.
