Objective:To explore the differences in social cognition between the first-episode schizophrenia and chronic schizophrenia, compare the differences between the two groups and the normal control group and analyze the correlation between social cognition and cognitive function and psychiatric symptoms. Methods:Patients with first-episode schizophrenia and chronic schizophrenia were included according to the International Classification of Diseases version 10(ICD-10),and healthy controls matched with gender, age, and education of the patient group were included.Social cognition was assessed using Facial emotion recognition, the Mayer-Salovey-Caruso Emotional Intelligence Tests,Picture Arrangement and Picture Completion(from WAIS-R)and Attribution Style Questionnaires.Social function was assessed using the Personal and Social Function Scale(PSP)and the Global Assessment Function(GAF).To assessed psychotic symptoms with the Positive and Negative Syndrome Scale(PANSS).Finally,63 patients were included in the first-episode schizophrenia group,60 in the chronic schizophrenia group,and50 in the healthy control group. We use SPSS 24.0 for statistical processing.Results:1.In the first-episode schizophrenia group, the chronic schizophrenia group and the healthy control group, the correct rate of Facial recognition in the first-episode schizophrenia group and the chronic schizophrenia group was lower than in the healthy control group(fearful?~2=16.009,P=0.000;no expression?~2=13.238,P=0.001;happiness?~2=12.057,P=0.002);in the reaction time, the first-episode schizophrenia group and the chronic schizophrenia group were worse than the healthy control group(fearful F=5.247,P=0.008;no expression F=11.692,P=0.000;happiness F=5.393,P=0.007).After multiple comparison, the results showed no difference between the first-episode schizophrenia group and the chronic schizophrenia group.Comparing the three groups, the first-episode schizophrenia group and chronic schizophrenia group in Picture Arrangement,Picture Completion and Emotional Management were worse than the healthy control group(F=11.259,P=0.00;F=10.002,P=0.00;F=4.712,P=0.02).After multiple comparisons, there was no difference between the first-episode schizophrenia group and the chronic schizophrenia group(P>0.05).2.There was no significant difference in attribution style between the first-episode schizophrenia group, the chronic schizophrenia group and the healthy control group(P>0.05).3.Comparison of social function, the PSP score of the healthy control group was better than the first-episode schizophrenia group and the chronic schizophrenia group(F=150.77,P=0.00).After multiple comparison, the PSP score of the first-episode schizophrenia group was higher than that of the chronic schizophrenia group(P=0.08);the GAF score of the healthy control group was better than the first-episode schizophrenia group and the chronic schizophrenia group(F=99.29,P=0.00).After a multiple comparison, there was no difference in the GAF score between the first-episode schizophrenia group and the chronic schizophrenia group(P=0.645).4.In the first-episode schizophrenia group, the correct rate of fearful was positively correlated with age(r=0.424,P<0.05),and positively correlated with GAF score(r=0.403,P<0.05).The correct rate of happiness emotion was positively correlated with age.(r=0.388,P<0.05),negatively correlated with PANSS positive score(r=-0.488,P<0.05).In chronic schizophrenia group, the correct rate of fearful emotion was positively correlated with age(r=0.463,P<0.05);the correct rate of happiness emotional was positively correlated with GAF score(r=0.467,P<0.05);fearful emotion response time was negatively correlated with duration of the disease(r=-0.485,P<0.05),and negatively correlated with PSP score(r=-0.485,P<0.05).5.In the first-episode schizophrenia group, the results of the Picture Arrangement were positively correlated with age(r=0.389,P<0.05),and the results of Picture Completion were positively correlated with PSP and GAF scores(r=0.453,r=0.386,P<0.05).In the chronic schizophrenia group:the Picture Arrangement was negatively correlated with the PANSS total score(r=-0.462,P<0.05),and the Emotional Intelligence was negatively correlated with the duration of the disease(r=-0.712,P<0.05),and the PANSS positive score(r=-0.821,P<0.05).6.In the first-episode schizophrenia group:the permanence-transience dimension of positive event was negatively correlated with the duration of the disease(r=-0.501,P<0.05),and the permanence-transience dimension of negative event was negatively correlated with the PASSN negative score(r=-0.349,P<0.05);in the chronic schizophrenia group:the intrinsic-external dimension of positive events was negatively correlated with PANSS positive score(r=-0.518,P<0.05),permanence-transience dimension was negatively correlated with the duration of the disease and PANSS positive scores(r=-0.450,r=-0.496,P<0.05),positively correlated with GAF score(r=0.456,P<0.05),and the overall-local dimension was negatively correlated with PANSS positive score(r=-0.471,P<0.05),the intrinsic-external dimension of negative events was positively correlated with GAF score(r=0.536,P<0.05).Conclusion:1.First-episode schizophrenia and chronic schizophrenia have damage in social cognition, the correct rate of Facial Emotion Recognition in first-episode schizophrenia and chronic schizophrenia was decreased, and the response time of facial emotion recognition was longer. The score of the Pcture Arangement and Picture Completion in first-episode schizophrenia and chronic schizophrenia patients was significantly lower than healthy control group. The scores of Emotional Management in first-episode schizophrenia and chronic schizophrenia were lower than healthy control group. There was no significant difference between first-episode schizophrenia and chronic schizophrenia.2.The social function of patients with first-episode schizophrenia is better than that of patients with chronic schizophrenia, but both are worse than healthy controls.3.The social cognitive impairment of patients with schizophrenia is related to their social function and psychotic symptoms. The more serious the social cognitive damage, the worse the social function;the more serious the psychotic symptoms, the more serious the social cognitive damage.
