Clinical Study On Treatment Of Lumbar Disc Herniation Of Liver And Kidney Deficiency Type By "Co-treatment Of Both Cervical And Lumber Vertebra" Theory

ObjectiveTo observe the clinical therapeutic effect of warm needling based on"co-treatment of both cervical and lumber vertebra " theory on lumbar disc herniation of liver and kidney deficiency type,and its influence on waist function and daily living ability of patients,so as to provide more ideas for clinical acupuncture and moxibustion treatment of this disease.MethodThis research adopted randomly controlled clinical studying me thod.From January 2018 to January 2019,Seventy LDH patients with deficiency of liver and kidney who met the inclusion criteria were randomly selected,and then randomly grouped into the neck-waist combined warm needling group and the simple lumbar warm needling group.The number of patients in each group was 35.The acupoints selected in the lumbar-neck combined therapy warm acupuncture group were Fengchi,Tianzhu,Jingbailao,Dazhu,Shenshu,Qihaishu,Dachangshu,Zhibian and Huantiao.All acupoints were bilateral.The acupoints selected in the lumbar warm acupuncture group were Shenshu,Qihaishu,Dachangshu,Zhibian and Huantiao on both sides.The operation methods of two groups:patients lay prone on the treatment bed,wore loose clothes,fully-exposed the neck and waist and buttocks.The doctor routinely disinfected the corresponding acupoints,surrounding skin and hands of doctor,then used 0.30*40mm,0.30*50mm,0.35*75mm(depending on different acupuncture points)tube needles of Huanqiu Brand needling straight,needling about 20-55 mm(depending on different acupuncture points).When the doctor felt the needle depressed,or the patient felt obvious acid swelling or tactile inductance at the needle site,the desired sensation was got.And then double edge cartons were padded between the needle bodies and the patient’s skin,with an extra layer of tin paper in the middle.The needle handles were covered with a 2.5 cm long stick ignited at the end.After about 30 minutes,the stick was burned,and the needle was removed after removing the ash.The specific operation method of the control group was the same as that of the treatment group.The treatment schemes were the patients treated once every 2 days,and 10 times as a course of treatment.At the same time,oswestry dysfunction index(ODI index),JOA score and X-ray lumbar scoliosis angle(Cobb angle)were used to evaluate the efficacy of the treatment.Result1.For he part of sex,age and course of disease,it was well matched between the groups(P>0.05).Before treatment,there was no significant difference in ODI index,JOA score and Cobb angle between the two groups(P>0.05).2.Both the ODI index of the two groups was apparently different after the treatment,contrasted with that of before(P<0.01).And compared with the simple lumbar warm acupuncture group,the ODI index of the "neck-waist combined therapy" warm acupuncture group decreased significantly(P<0.01).Both of them could reduce the ODI index of patients,and the effect of "neck and waist combined therapy" warming acupuncture group was better in improving the daily activity function of patients.3.Both the JOA scores of the two groups were apparently different after the treatment,contrasted with that of before(P<0.01).And compared with the simple lumbar warm acupuncture group,the score of the "cervical and lumbar combined therapy" warm acupuncture group increased significantly(P<0.01).It showed that both of them could increase JOA score,and the effect of "neck and waist combined treatment" warm acupuncture group was better in improving the symptoms such as pain,sensory and motor signs of patients.4.Both the Cobb angle between the two groups was apparently different after the treatment,contrasted with that of before(P<0.01).Compared with the lumbar warm moxibustion group,the Cobb angle of the lumbar warm moxibustion group decreased apparently(P<0.05),which showed that the lumbar scoliosis improvement of the lumbar warm moxibustion group was better than that of the lumbar warm moxibustion group.5.After treatment,the curative effect of the two groups was compared,and there was an apparent difference between them(P<0.01).The total effective rate of the "cervical lumbar combined therapy" warm acupuncture group was 94.29%,higher than that of the simple lumbar warm acupuncture group(77.14%,P<0.05);the markedly effective rate of the "cervical lumbar combined therapy" warm acupuncture group was 82.86%,higher than that of the simple lumbar warm acupuncture group(51.43%,P<0.01).Conclusion:Routine lumbar warm needling is also effective in the treatment of lumbar disc herniation with deficiency of liver and kidney,but "combined treatment of neck and lumbar" warming needling is more effective in relieving lumbar pain,improving daily activity function and improving lumbar scoliosis.The mechanism may be related to the effective restoration of spine biomechanics balance by warming acupuncture and moxibustion based on co-treatment of both cervical and lumber vertebra.
