ObjectivesPreparation renal allograft model by using male New Zealand white rabbits,we stroge the rabbit renal through anterograde continuous perfusion which start from renal artery in low temperature and through retrograde continuous perfusion which start from renal vein in low temperature.then they were performed the renal allograft.After the renal transplantation surgery to determine serum inosine and serum urea creatine from New Zealand white rabbits?renal tissue pathology,epidermal growth factor(EGF)in the kidney tissues and myeloperoxidase(MPO)expression level,discussed the different perfusion means impact on the quality of donor renal,to investigate renal preservation by a novel method and Increasing utilizati of extended criteria donor such as renal artery variations for clinical practice.MethodsAdult healthy male New Zealand rabbits were selected as donors and recipients of kidney transplantation,with a body weight of 2.0-3.0kg.A total of 60 New Zealand rabbits were numbered from 1 to 60,and were divided into two groups using A completely random design(random number table method).The anterograde continuous hypothennia group(group A)and the reverse continuous hypothermia group(group B)each had 30 rabbits.Then the white rabbits were randomly divided into two groups(donor group and recipient group),15 rabbits in each group.The kidney of the donor in group A and group B was transplanted into the recipient in situ after 15min of warm ischemia according to the method of different groups for 2 hours,and the recipient's right kidney was removed after the completion of transplantation.Anti-rejection with cyclosporine A after the operation,the serum creatinine level and urine volume of the New Zealand rabbits in the two different perfusion groups were compared on the first,third and fifth days after the operation,and the renal transplantation histopathology(HE staining),MPO and EGF expression in the renal transplantation tissue were observed at fifth days after the operationResults1.Serum creatinine changesThe serum creatinine values of group A(anterograde perfusion group)on the first day,the third day and the fifth day after surgery were(323.89±19.15)umol/L,(203.34±14.02)umol/L,(121.51±8.76)umol/L,respectively.The serum creatinine values of group B(reverse perfusion group)on the first day,the third day and the fifth day after surgery were(335.11 ± 18.62)umol/L,(212.03±9.66)umol/L and(125.94±7.35)umol/L,respectively.Compared with the two groups,there was no significant difference in the postoperative serum creatinine between the retrograde perfusion group and the anterograde perfusion group,P>0.05,and the difference was not statistically significant.2.Urine volume changesThe urine volume of group A(anterograde perfusion group)on the first,third and fifth days after surgery was(96.36±9.36)ml,(129.91 ± 10.72)ml and(168.40 ± 12.34)ml,respectively.The urine volume of group B(reverse perfusion group)on the first,third and fifth days after surgery was(91.89± 8.30)ml,(123.78±10.79)ml and(158.23±16.46)ml,respectively.Compared with the two groups,there was no significant difference in postoperative urine volume between the retrograde perfusion group and the anterograde perfusion group,P>0.05,and the difference was not statistically significant.3.Histopathology of transplanted kidney(HE staining)Two groups of transplant renal pathological HE staining results showed that after the fifth day,groupA(anterograde perfusion group)and groupB(retrograde perfusion group)graft glomerular and renal tubular structure is close to normal,less interstitial edema and inflammatory cells infiltration,retrograde perfusion group organization structure and anterograde postoperative perfusion group there was no obvious difference in organizational structure comparison,P>0.05,and the difference was not statistically significant.4.EGF expression was detected by immunohistochemistry of the transplanted kidney.The EGF positive expression rate of group A(anterograde perfusion group)on the fifth day after surgery was(23.27±4.48)%,respectively.The EGF positive expression rate of group B(reverse perfusion group)on the fifth days after surgery was(20.70±3.34)%,respectively.Compared with the two groups,there was no significant difference in the EGF expression between the retrograde perfusion group and the anterograde perfusion group,P>0.05,and the difference was not statistically significant.5.MPO detection of transplanted kidneyThe MPO expression of group A(anterograde perfusion group)on the fifth day after surgery was(0.474±0.07)U/g,respectively.The EGF positive expression rate of group B(reverse perfusion group)on the fifth days after surgery was(0.425±0.08)U/g,respectively.Compared with the two groups,there was no significant difference in the MPO expression between the retrograde perfusion group and the anterograde perfusion group(P>0.05),and the difference was not statistically significant.ConclusionThe Retrograde perfusion method has the same protective effect on the transplanted kidney as the anterograde perfusion method.Retrograde perfusion can be used as an effective remedy for renal insufficiency caused by renal artery malformation,stenosis and so on.
