Object:To investigate the correlation between umbilical arterial blood lactate concentration and pH(pH value)and base excess(BE value)at birth;to study the evaluation value of umbilical arterial blood lactate concentration in neonatal asphyxia and neonatal short-term adverse outcomes.Methods:A retrospective analysis of the distribution of lactate concentration,PH value and BE value of umbilical arteries in neonates from October 2017 to October 2018 in the Department of Obstetrics and Gynecology,Dayi Hospital.Pearson correlation statistical method was used to analyze the correlation between umbilical arterial blood lactate concentration and pH value,BE value.The ROC curve area was used to analyze the value of lactate concentration in neonatal asphyxia and short-term adverse prognosis in neonates.Results:1 The median blood lactate concentration of 1355 neonates in this study was 4.95(3.5,6.6)mmol / L,the median value of pH was 7.28(7.21,7.32),the median value of BE was-4.80(-6.9,-3.1)mmol/L.2 There was a significant negative correlation between umbilical arterial blood lactate concentration and pH,BE(P < 0.001).3 This study showed that with the aggravation of acidosis,the cutoff value of umbilical arterial blood lactate concentration calculated by ROC curve gradually increased,and the optimal cutoff value of neonatal asphyxia in lactate concentration was12.3mmol/L(P <0.001),the optimal cutoff value of lactate concentration in predicting short-term adverse prognosis in neonates was 7.35 mmol/L(P < 0.001).Conclusions:1 The umbilical arterial blood lactate concentration gradually increased with the severity of neonatal acidosis.2 The umbilical arterial blood lactate concentration could be used as an effective reference index for the evaluation of neonatal asphyxia during childbirth,and could also predict the short-term adverse prognosis of neonates.
