Objective: To explore the efficacy and influencing factors of endoscopic three ventriclulostomy(ETV)in the treatment of hydrocephalus.Methods: Retrospective study was conducted on 26 hydrocephalus patients treated with ETV in the department of neurosurgery of Hebei General hospital from August 2013 to August 2017.Classification according to the primary disease: 1.7 cases of middle cerebral aqueduct obstruction;2.The aneurysmal subarachnoid hemorrhage(aSAH)14 cases;3.The intraventricular hemorrhage(IVH)in 5 cases.To observe the postoperative efficacy of the 26 patients and follow up for 10-42 months,and summarize the experience through analysis and research.Results: The overall effective rate of ETV was 73% in 26 cases of hydrocephalus.Middle cerebral aqueduct obstruction was effective in 6 cases,with an effective rate of 85.7%.ASAH was effective in 10 cases,with an effective rate of 71.4%.IVH was effective in 3 cases and the effective rate was 60%.During the follow-up,4 patients with recurrent hydrocephalus underwent ventriculo-peritoneal shunt(VPS).Complications: 1 case of middle cerebral aqueduct obstruction hydrocephalus developed oculomotor paralysis postoperative.Conclusion: ETV is safe and effective in the treatment of hydrocephalus.It can also achieve significant results in some patients with hydrocephalus after aSAH and IVH,and when necessary,combined choroid plexus cauterization(CPC)can also achieve good results in some patients with communicating hydrocephalus.
