Objective:To summarize the pathogenesis,clinical characteristics,differential diagnosis,treatment and related progress of type 2 myocardial infarction,and to analyze the diagnosis and treatment of coronary artery spasm.Methods:Retrospectively analyze a T2 MI case caused by coronary artery spasm and review the current literature.Results:The 53-year-old,male patient was admitted to hospital for intermittent chest pain 2 years,recurrent 3 hours.ECG showed depressed ST segment in leads II III av F,elevated ST segment in leads I av L V1-V6 and high sensitive troponin I was 8.9500 ng/m L.Coronary angiography confirmed that there was no obvious stenosis,but vasospasm in the middle segment of RCA.The motion in left ventricular apex got weak.IVUS revealed plaque in the LAD ostium and proximal segment,no plaque rupture,dissection and thrombus.It was in accordance with universal definition of T2 MI according to the patient's symptoms and auxiliary examination.The patient got better and was discharged after taking antiplatelet agent,coronary vasodilator,antispasmodic,statin,and implanting pacemaker.Conclusion:T2MI caused by coronary artery spasm is common.Active drug therapy is supported by current clinical studies in patients with T2 MI.It is of great significance to accurately judge the pathophysiological mechanism and classification of myocardial infarction in diagnosis,treatment and fully evaluating prognosis.
