Objective: Pancreatic ductal adenocarcinoma is the most common pathological type of pancreatic cancer.It was known as bad malignancy and poor prognosis.In recent years,with the progress of surgery and comprehensive therapy,the diagnosis and treatment of pancreatic ductal adenocarcinoma has been improved,but the overall prognosis is still not optimistic.Now,there is no consensus in the world on the prognostic factors of pancreatic ductal adenocarcinoma.In this study,we combine the research hot area nowadays to analyze the relationship between clinicopathological factors and prognosis of pancreatic ductal adenocarcinoma.Materials and Methods: Retrospectively analyzed the clinical and pathological data of pancreatic ductal adenocarcinoma surgical treatment in the Eastern Hospital of the Fourth Hospital of Hebei Medical University from October 2010 to September 2018.Including: sex,age,location of tumors,size of tumors,if combined organ resection,if combined vascular resection or reconstruction,if extended lymph node dissection,number of lymph node dissection,number of lymph node metastasis,TNM stage,degree of differentiation of tumors,nerve invasion,vessel invasion,complications,CEA,CA199.The patients' OS and DFS were obtained by telephone follow-up,case review or re-hospitalization.Kaplan-Meier method was used for univariate analysis and drawing survival curve.Log-rank test was used for comparison of survival curves among groups,and COX regression analysis was used for multivariate analysis.Results:1.The median OS of 101 pancreatic ductal adenocarcinoma patients was 14.6 months.The 1-year,3-year and 5-year survival rates were 60.2%,22.4% and 12% respectively.The median DFS was 10.1 months.The 1-year,3-years,5-year disease-free survival rate was 42.9%,15.0% and 10%.2.In univariate analysis,tumor size,TNM stage,CA199 value,degree of differentiation,nerve invasion,vessel invasion were significant factors affecting OS of patients with pancreatic ductal adenocarcinoma after operation;age,tumor size,TMN stage,CA199 value,degree of differentiation,nerve invasion,vessel invasion were significant factors of DFS.3.Multivariate analysis: Tumor size,degree of differentiation,nerve invasion,and vessel invasion were independent risk factors affecting OS;age,tumor size,nerve invasion,and vessel invasion were independent risk factors for DFS.Conclusion: Tumor size,differentiation degree,nerve invasion and vessel invasion are independent risk factors affecting OS.Age,tumor size,nerve invasion,and vessel invasion were independent risk factors for DFS.
