Object:The purpose of this study is to analyze the relationship between syndrome types, sputum color and pulmonary infection, and severity of AECOPD patients,to determine whether there is pulmonary infection and severity of infection can be judged by sputum color, and to summarize The correlation between TCM syndrome differentiation and various inflammatory indexes, lung function grading, sputum pathogens and other test results provides assistance for the patient's condition judgment and diagnosis and treatment in clinical work. Methods:A retrospective study was conducted to collect clinical data of 162AECOPD patients who met the inclusion criteria from December 2016 to December2018 in the Department of Respiratory Medicine,Affiliated Hospital of Chengdu University of Traditional Chinese Medicine.Reference was made to the dialectical criteria in the Guidelines for Diagnosis and Treatment of Chronic Obstructive Pulmonary Disease(2011 edition).Phlegm-dampness obstruction of lung and phlegm-heat obstruction of lung were grouped and the severity of the disease was graded.Scoring with standard chromatographic comparison, and determining the relevant inflammation index and sputum pathogenic characteristics of each group;and statistically analyzing the correlation between TCM syndrome type, sputum color score and lung pathogenicity and severity of disease Distribution. Results:For AECOPD patients,There were significant statistical differences in the levels of IL-6 and PCT between patients with phlegm-dampness obstruction of lung and patients with phlegm-heat obstruction of lung(P<0.05).The comparison of pulmonary function grading between different syndrome types and severity of airflow restriction in AECOPD patients has statistical significance(?2=21.897,P<0.05).The correlation analysis showed that the sputum color score was correlated with the levels of IL-6 and PCT in blood(P<0.05,0<R<1).There was a significant positive correlation between different sputum color groups and the severity of airflow limitation in acute exacerbation of chronic obstructive pulmonary disease(P<0.05,0<R_S<1).According to the principle of color grouping,,There was a statistically significant difference after statistical analysis in the proportion of Gram-negative bacteria and Gram-positive bacteria in the above-mentioned sputum color grouping(P<0.05).Conclusion:The pathogens isolated from AECOPD patients are mainly Gram-negative bacteria, reflecting the overall classification characteristics of pathogenic bacteria. The level of airway inflammation in AECOPD patients with phlegm-heat obstruction of lung was significantly higher than that in patients with phlegm-dampness obstruction of lung, and the condition was more serious in the group of phlegm-heat obstruction of lung. The severity classification and airway inflammation of patients with yellow phlegm were more serious. The proportion of Gram-negative bacteria and Gram-positive bacteria varied with sputum color scores, suggesting that different sputum color may have different bacterial spectrum. This study can help clinicians to judge the patient's condition and adjust the diagnosis and treatment plan. This study is of great significance to guide medication by experience and to improve the curative effect.
