Objective : Analyze the incidence,pathogens,and risk factors of early-onset sepsis(EOS)and the late-onset sepsis(LOS)of extremely low birth weight(ELBW)and the very low birth weight(VLBW)infants in Children's Hospital of Chongqing Medical University.Method: Data from January 1,2012 to December 31,2017 was collected and divided into EOS,LOS and control group.And it was used to make a retrospective study of the incidence,pathogen,risk factors and adverse prognosis.Results: Newborns of 777 were included,of whom 106 were EOS,244 were LOS.Strains of 9 were cultured in EOS.The organisms most frequently involved in EOS is Escherichia coli,which accounts for 44.4%.130 strains of LOS were cultured,Klebsiella pneumoniae is the most common etiology(39.2%),additional pathogens like fungi,accounted for18.5%.In a multivariate logistic regression analysis,premature rupture of membranes(PROM)(odds ratio(OR):2.23,95% confidence interval(CI):1.30-3.81),initial resuscitation(OR 3.25,95%CI 1.60-6.60),mechanicalventilation at the first day of birth(OR 4.39,95%CI 2.31-8.35),lower birth weight(OR 0.42,95%CI 0.28-0.62)were shown to be risk factors of EOS(P< 0.05).In a multivariate logistic regression analysis,mechanical ventilation(OR 1.72,95%CI 1.14-2.61),lower birth weight(OR 0.47,95%CI 0.36-0.62)were shown to risk factors of LOS(P<0.05).Cesarean section(OR 0.50,95%CI 0.33-0.75)were shown to be protective against LOS(P<0.05).Conclusion:PROM,chorioamnionitis,asphyxia,initial resuscitation,mechanical ventilation at the first day of birth contribute to the risk factors of EOS and mechanical ventilation contribute to the risk factors of LOS.
