?Objection? According to related research,the global “sepsis-like” syndrome is one of the vital pathophysiologic changes in post resuscitation patients.This study aims at observing the inflammatory changes in the complicate pathophysiology of post myocardial arrest dysfunction.At the same time we wonder is there a regulatory effect of ghrelin to inflammation,and whether ghrelin is protective to post myocardial arrest dysfunction.?Method? The whole study was divided into two parts.Part 1,250 g – 300 g healthy SD rats were suffered transcutaneous electrical epicardium stimulation to be caused 6-minutes cardia arrest(CA)and successfully resuscitated.Rats were than randomly and respectively executed at 1h,3h,6h,24 h after they were resuscitated.The hemodynamic outcomes of each rats in each time points were recorded,and the heart tissues were harvested to test the expression of NLRP3 inflammasome pathway.Part 2 250 g – 300 g healthy rats were randomly allocated to three group:(1)SHAM group,(2)GH group,(3)NS group.After successfully achieved of CA and ROSC,all rats were executed 6h after ROSC,heart tissues and blood samples were collected for further test.3 of each group were taken TTC test to evaluate the infarction of hearts.3 of each group were taken HE staining and TUNEL staining to observe the damage of heart tissues brought by CA and ROSC.The rest samples were taken following tests:(1)using ELISA kits to test the myocardium markers in blood serum.(2)using ELISA kits to test the level of inflammatory cytokines and MPO in heart tissues,(3)using commercial kits to test the level of MDA and SOD in heart tissues,(4)using western blot to test the expression of NLRP3 inflammasome pathway.?Results? In Part1,the hemodynamic outcomes are getting worse after the rats get ROSC,however,when it comes to 24 h,there were no significant differences between groups.The NLRP3 inflammasome pathway is significantly expressed in heart tissues and getting increased with time goes in 6h,the level of expression in 24 h is significantly lower than 6h.In Part 2,(1)GH significantly reduced the infarction size of hearts compared with NS group.(3)GH reduced the damage of heart tissues and lower the apoptosis index compared with NS group.(4)GH lower the production of myocardium markers in blood serum compared with NS group.(5)GH reduce the production of inflammatory cytokines and lower the activity of MPO compared with NS group.(6)GH reduced the production of MDA and improved the activity of SOD compared with NS group.(7)GH downregulate the expression of NLRP3 inflammasome pathway.?Conclusion? Inflammatory change is involved in the complicate pathophysiology of post myocardial arrest dysfunction.Ghrelin has a regulatory effects on post resuscitation inflammation,and Ghrelin is able to protect the post-resuscitating heart tissues,this protective effects may have relationship with its anti-effects to post-resuscitation inflammation.
