Objectives The concentration of bile acid subtypes in serum of gallstone,cholesterol polyp and gallbladder adenoma in normal control group and gallbladder stone,gallbladder polyp and gallbladder adenoma were analyzed,and the composition and concentration of bile acid subtypes in serum of gallbladder stone,cholesterol polyp and gallbladder adenoma were studied,so as to find out the subtypes of bile acid for disease differentiation and better guide clinical treatment and prevention.Methods From September 2017 to December 2018,46 patients with cholecystolithiasis or protuberant lesions of gallbladder were treated surgically in Hebei People's Hospital.During the same period,10 patients admitted to hospital for non-gallbladder diseases were treated by surgery as controls.The patients were divided into gallstone group,cholesterol polyp group and gallbladder adenoma group according to the pathological results.All sera were monitored by liquid chromatography tandem mass spectrometer(LC-MS).Statistical analysis with SPSS 21.0 software was used to test the normality of the data of each group.The data of each group in this study did not obey the normal distribution.Four-table chisquare test was used to test the counting data(such as gender),and the age of each group was analyzed by non-parametric test with multiple independent samples.Median(quartile spacing)(M(QR))was used to describe the measurement data in the study.The variances of each group were uneven or(and)skewed distribution.Non-parametric tests were conducted with multiple independent samples.Statistical analysis was carried out for each statistic,and the statistical results were analyzed in combination with clinical data.If P < 0.05,the difference is statistically significant.Results In this study,56 patients were included,of which 46 were in case group(cholesterol polyp group,gallstone group,gallbladder adenoma group)and 10 were in control group.The gender of each group was tested by four-grid chi-square test,and the age of each group was tested by non-parametric test of multiple independent samples,P > 0.05.There was no significant difference between the two groups.The results showed that the serum concentrations of CA,UDCA,GCA,GCDCA,GUDCA,TCA,TCDCA and CDCA in the control group were significantly different from those in the medical records group(P < 0.05).The serum concentrations of CA,UDCA,GCA,GCDCA,GUDCA,TCA,TCDCA and CDCA in stone group and polyp group,polyp group and adenoma group,adenoma group and stone group were not significantly different(P > 0.05).The concentration of DCA,GDCA and LCA in serum of control group was significantly different from that of bile acid in stone group and adenoma group(P < 0.05).There was no significant difference in serum DCA,GDCA and LCA between the control group and polyp group,stone group and polyp group,polyp group and adenoma group,adenoma group and stone group(P > 0.05).The serum TLCA concentration in the control group was significantly higher than that in the stone group and polyp group(P < 0.05).The serum TLCA levels in the control group and adenoma,stone group and polyp group,polyp group and adenoma group,adenoma group and stone group were compared by pairs(P > 0.05),with no statistical significance.The concentrations of GLCA,TDCA and TUDCA in serum of bile acid in control group and stone group were compared by two methods(P < 0.05),and the difference was statistically significant.The serum concentrations of GLCA,TDCA and TUDCA in control group and adenoma group,control group and polyp group,stone group and polyp group,polyp group and adenoma group,adenoma group and stone group were compared by two methods(P > 0.05),with no statistical significance.Conclusions(1)The concentration of bile acid in serum of gallstone,gallbladder polyp and gallbladder adenoma was different from that of control group,and the composition of bile acid subtypes in serum of each case group was different.Considering the different pathogenesis and metabolic characteristics of gallstone,gallbladder polyp and gallbladder adenoma,the concentration of bile acid in serum of each case group was abnormal in varying degrees.Especially,the concentration of hydrophobic bile acid CA,GCDCA and CDCA in gallbladder stones,polyps and adenomas of gallbladder is significantly higher than that in control group.Considering that hydrophobic bile acid plays an important role in the pathogenesis of gallbladder diseases;(3)The concentration of serum GLCA,TDCA and TUDCA in gallbladder stones group is different from that in control group,which has guiding significance for the study of the causes and treatment of gallbladder stones;(4)This study The results showed that there was no difference in serum bile acid concentration among gallstones,gallbladder polyps and gallbladder adenomas.No indices were found to differentiate gallstones,gallbladder polyps and gallbladder adenomas.Figure0;Table4;Reference 119.
