| Objective: To understand the changes of tinnitus after tympanoplasty,to investigate the causes of tinnitus after tympanoplasty and the outcome of tinnitus and different tympanoplasty,postoperative hearing changes,chronic otitis media classification,eustachian tube status and postoperative sleep The relationship between quality and other factors.Methods: A total of 110 patients with chronic otitis media who underwent tympanoplasty from September 2015 to May 2016 were selected from the Affiliated Hospital of Qingdao University.45 patients with tinnitus before operation were divided into preoperative tinnitus group(n=45)and preoperative no tinnitus group(n=65)according to whether they had tinnitus before operation.The patients were followed up for 3 months.Hearing conditions,patients with tinnitus before and 1 week and 3 months after surgery were filled out with a tinnitus score scale.The scores were assessed by the THI,tinnitus severity and efficacy evaluation scale as criteria.Finally,the measurement data were taken as mean()± standard deviation(SD),and the data variance was tested by t test;the variance was not checked and the t’ test was performed.Result: Of the 110 patients with chronic otitis media,45 patients with chronic otitis media with tinnitus were enrolled.After 43 patients with informed consent,the THI,tinnitus severity and efficacy evaluation scale were filled out before the operation.Among them,the incidence of tinnitus in chronic otitis media was 40.9%,of which 43 were involved in the survey.The preoperative tinnitus disability scale(THI)score: functional score was17.10±4.1,and affective score was 19.32±3.20,severity.The score was 5.56±1.32 points;preoperative tinnitus severity and efficacy evaluation scale analysis: according to the severity of tinnitus,there were 14 cases at grade 1,16 at grade 2,5 at grade 3,and 6 at grade4.There were 2 cases at level 5,and the proportion of patients with severity above level 2was as high as 67.4%.68 patients had tinnitus 1 week after operation,the incidence rate was62.97%,the tinnitus disability scale(THI)score: functional score was 18.46 ± 3.41 points,emotional score was 20.13 ± 2.56 points,severity score was 6.12 ± 1.28 points Tinnitus severity and efficacy evaluation scale analysis: According to the severity of tinnitus,there are 26 cases of grade 1,22 cases of grade 2,8 cases of grade 3,7 cases of grade 4,5 cases of grade 5,patients The proportion of severity above level 2 is as high as 61.8%.At 3 months after operation,2 patients had no dry ear,32 of them were tinnitus,the incidence rate was30.19%,and the tinnitus disability scale(THI)score: functional score was 8.62±2.05,and the emotional score was 7.13±.1.35 points,the severity score was 2.37±0.69 points;the severity of tinnitus and the evaluation of therapeutic effect scale: according to the severity of tinnitus,there were 18 cases in grade 1,11 cases in level 2,and 2 cases in level 3,level 4There were 1 case,0 cases of 5 cases,and the proportion of patients with severity above level 2 was 43.75%.Multivariate analysis of eustachian tube status,postoperative hearing improvement,and postoperative sleep quality were associated with postoperative tinnitus outcomes(P<0.05),while the nature,surgical style,and intraoperative use of chronic otitis media There was no correlation between postoperative tinnitus outcomes(P>0.05).The tinnitus in the follow-up group at 3 months after operation was significantly improved compared with the 1-week follow-up group,and the difference was statistically significant(P<0.05).Conclusion:1.One week after tympanoplasty,tinnitus is worse than preoperative,which is related to postoperative operative tamponade,postoperative sleep quality,postoperative patient’s mental state,and the type of chronic otitis media,the procedure used,and The use of the intraoperative drill does not matter.2.At 3 months after operation,tinnitus was significantly relieved compared with preoperative and postoperative 1 week.The recovery of tinnitus after eustachian tube patency was better than that of eustachian tube.The postoperative hearing loss was better than that of postoperative hearing loss.Hearing improvement was not obvious or decreased;postoperative sleep quality was better after surgery.3.There was no significant correlation between acute tinnitus after different types of tympanoplasty,but the improvement of hearing and tinnitus in patients with type I tympanoplasty was more significant than the other four types. |
