Objective:Cholangiocarcinoma is a malignant tumor of the biliary system with low cure rate,high mortality and poor long-term survival rate.With the in-depth study of microRNAs,small non-coding RNAs,in the medical community,many scholars have confirmed that microRNAs play an important role in the development of many malignant tumors.This study aimed to investigate the relationship between the expression level of miR-224 in cancer tissues and adjacent tissues of patients with cholangiocarcinoma and the relationship between its expression and age,sex,tumor differentiation,lymph node metastasis and tumor TNM staging to clarify its clinicopathological significance in patients with cholangiocarcinoma.We followed up and compared the postoperative survival time of patients with cholangiocarcinoma with different miR-224 expression levels,and analyzed its relationship with other clinical indicators,then confirmed its clinical significance in evaluating the prognosis of patients with cholangiocarcinoma.Methods:Thirty patients with newly diagnosed cholangiocarcinoma,reasonable age distribution,complete clinical and pathological data and confirmed by histopathology were selected according to the inclusion and exclusion criteria.The cancer tissue and adjacent tissues were separately taken and kept for subsequent experiments.After total RNA extraction and reverse transcription to obtain cDNA,RT-PCR was used for the detection of miR-224 expression in cancer tissues and adjacent tissues of patients with cholangiocarcinoma,and statistical methods were used to compare them with cancer tissues and adjacent tissues.The expression in the tissues was analyzed to determine whether the difference in expression was statistically significant.The complete clinical pathology data were collected and statistical methods were used to compare the relative expression of miR-224 in cholangiocarcinoma and the clinicopathological features such as age,sex,tumor differentiation,lymph node metastasis and tumor TNM staging.We investigated the survival of 30 patients with cholangiocarcinoma by telephone follow-up,recorded their postoperative survival,performed survival analysis to map survival curves,and established a statistical model to analyze the influence of various factors on postoperative survival time,to determine the value of microRNA-224 in determining the prognosis of patients with cholangiocarcinoma.Results:The relative expression of miR-224-5p in cancer tissues of patients with cholangiocarcinoma was(10.23±0.8861),and the relative expression in adjacent tissues was(0.4734±0.0402).The expression level of miR-224-5p in cholangiocarcinoma tissues was significantly higher than that in adjacent tissues(P<0.05).Thirty patients were divided into miR-224-5p high expression group(15 cases)and low expression group(15cases)with the median expression level of miR-224-5p.The results showed that the positive rate of lymph node metastasis in miR-224-5p high expression group was higher than that in miR-224-5p low expression group(P<0.05).In addition,the expression level of miR-224-5p was closely related to the TNM pathological stage of patients.The proportion of Phase I and II patients in miR-224-5p low expression group was significan-tly higher than that of miR-224-5p high expression group(P<0.05).There were no significant differences in clinicopathological factors such as age,gender,and tumor differentiation between the two groups(P>0.05).The median survival time was 22.54months(95%CI 16.76-28.32 months)in the miR-224-5p high expression group,and32.23 months in the low expression group(95%CI 29.86-34.60 months),the survival time of the latter was significantly longer than the former(X~2=9.169,P<0.05).The Cox proportional hazard regression model was analyzed and the results showed that high expression of miR-224-5p is a factor affecting the prognosis of patients with cholangiocarcinoma independent of other clinicopathological features.Conclusion:microRNA-224 has a potential oncogene effect in cholangiocarcinoma;high expression of miR-224 has a negative effect on the prognosis of patients with cholangiocarcinoma.High expression of microRNA-224 is a marker that indicates the poor prognosis of cholangiocarcinoma independent of other clinicopathological features.Its expression level can be used as an important indicator in the prognosis system of cholangiocarcinoma.
