A Comparative Study Of Scarf And Ludloff Osteotomy In The Treatment Of Moderate And Severe Hallux Valgus

Objective: By compare the effect of Scarf osteotomy and Ludloff osteotomy i n the treatment of moderate and severe hallux valgus,and to understand the differ ence between Scarf osteotomy and Ludloff osteotomy in the treatment of moderate and severe hallux valgus.Methods: The patients who underwent Scarf osteotomy and Ludloff osteotomy in Qingdao Municipal Hospital from August 2015 to December 2017 due to hallu x valgus were divided into two groups: Scarf osteotomy group and Ludloff osteoto my group.The follow-up of HVA,IMA,tibial sesamoid displacement,first metatar sal length,VAS score and AFAS score before and after operation was observed an d recorded.Data,using statistical software and methods for data analysis.Results: The length of the first metatarsal bone in Ludloff osteotomy group w as(59.23±4.55mm)significantly longer than that in Scarf osteotomy group(61.42±4.64mm)(P < 0.05);there was a significant difference between Ludloff osteotomy gr oup and Scarf osteotomy group in tibial sesamoid displacement(2.11±0.82)and tibial sesamoid displacement(0.89±0.54)(P < 0.05);there was a significant difference b etween Ludloff osteotomy group and Scarf osteotomy group.There were 1 case of metastatic metatarsal pain and 1 case of complications of thumb stiffness in Sc arf osteotomy group,1 case of complications of thumb stiffness in Scarf osteotom y group,There were 1 case of metastatic metatarsal pain and 1 case of complicati ons of thumb stiffness in Scarf osteotomy group,1 case of complications of thum b stiffness in Scarf osteotomy group,and the VAS score of Ludloff osteotomy gro up was lower than that of Scarf osteotomy group in the same period,the differen ce was significant at 1 week and 1 month after operation(P < 0.05),3 months a nd 6 months after operation in two groups.There was no significant difference in VAS scores between the two groups(P > 0.05),and the expected results of painfree were achieved one year after operation.AOFAS scores of the two groups con tinued to increase,and the AOFAS scores of the Ludloff osteotomy group were hi gher than those of the Scarf osteotomy group at the same time,the difference was statistically significant(P < 0.05).Conclusion: Compared with Scarf osteotomy,Ludloff osteotomy is superior to Scarf osteotomy in early pain relief.It is helpful for patients to perform function al exercises assisted by heels in the early stage,but Scarf osteotomy is superior to Ludloff osteotomy in the correction of tibial sesamoid displacement.Ludloff osteo tomy is faster and more effective than Scarf osteotomy in restoring normal foot fu nction.But LudLoff osteotomy is more prone to shorten the first metatarsal lengththan Scarf osteotomy,which is why Ludloff osteotomy is more prone to metastati c metatarsal pain than Scarf osteotomy in terms of complications.In view of the definite surgical effect,the two methods are worth popularizing.
