ObjectiveTo investigate the intrinsic motion diffusion-weighted imaging?IVIM-DWI?and dynamic contrast enhanced MR?DCE-MRI?scanning partial quantitative parameters for prostate cancer?Prostate cancer,The diagnostic value of PCa?,combined with the formation of different scanning protocols to find the best screening protocol.At the same time,combined with PI-RADS V2 scoring standard,the application value of PI-RADS V2 scoring system in PCa multi-parameter MRI diagnosis was studied.Methods A total of 70 patients with abnormal prostate specific antigen?PSA?who were admitted to the Ordos Central Hospital from October 2016 to February 2019,and 70 patients who met the study criteria were enrolled.PCa group?34 cases?and non-PCa group?36 cases?.Prostate IVIM-DWI,DCE-MRI and conventional sequence scans were performed with GE 3.0T MR.The TIC curve of DCE-MRI and the parameter capacity transfer constant(Ktrans),rate constant(Kep),extravascular extracellular volume constant?Ve?,pure diffusion coefficient?D?,pseudo-dispersion coefficient?D*?were obtained by post-processing.The value of the perfusion score?f?was analyzed and statistical analysis was performed.The above three examination forms were combined to form the following three protocols:1T2WI+DCE-MRI2T2WI+IVIM-DWI3T2WI+DCE-MRI+IVIM-DWI,and the diagnostic specificity,sensitivity and AUC of each protocol were compared with pathological results.Cases were scored according to the PI-RADS V2 scoring system,and the accuracy of the scoring criteria was statistically analyzed against pathological results.Result 1.The D values of the PCa group and the non-PCa group are?0.55±0.09?×10-3mm2/s,?0.77±0.18?×10-3mm2/s,and the f values are 0.205?0.195-0.254?and 0.188?0.17475-0.196?,Ktransrans values were 0.508?0.4185-0.793?/min,0.1985?0.147-0.2995?/min,and Kepp values were 1.66±0.96/min and 0.99±0.59/min,Statistical significance?p<0.05?;D*values were 0.815?0.7425-0.983?×10-3mm2/s,0.926?0.87225-1.003?×10-3mm2/s,respectively,and the Ve values were 0.28±0.12,0.27±0.13,There was no significant difference between the two comparisons?p>0.05?.2?Among the 2 kinds of examination schemes,the diagnostic sensitivity and specificity of scheme 3 are the highest,respectively 93.5%,92.31%,and the AUC values of the three groups are 0.882,0.965,0.981,and scheme 3 and scheme 1,scheme The AUC comparisons between the two groups were statistically significant?p<0.05?.3?The consistency of the PI-RADS V2 scores of the two observers was good?Kappa value=0.710?.Conclusion 1.?The D value of the PCa group was smaller than that of the non-PCa group.The Ktrans,Kepp and f values of the PCa group were higher than those of the non-PCa group,and the difference was statistically significant.There was no significant difference in the D*value and the Ve value between the two.2?The diagnostic efficiency of each parameter from low to high is Ktrans?AUC=0.871?,D?AUC=0.845?,f?AUC=0.832?,Kep?AUC=0.720?,D*?AUC=0.611?,Ve?AUC=0.510?.3?IVIM-DWI and DCE-MRI can independently and effectively diagnose PCa.The combination of the two techniques is more conducive to the diagnosis of PCa,and at the same time,the lesion can be analyzed from multiple angles.4?PI-RADS V2 has a high diagnostic value for PCa and is clinically practical.
