Part ? Value of MRI to the Diagnosis of Viral EncephalitisPurpose:To investigate the value of conventional magnetic resonance imaging?MRI?and diffusion weighted imaging?DWI?to the diagnosis of viral encephalitisMethods:We collected 40 patients,who were diagnosed with viral encephalitis from June 2012 to June 2015.All of the patients included were with MRI abnormalities,needing conventional MRI and 35 of them needing enhanced scan.We divided the 40 patients into three groups according to the number,range and signal intensity of lesions displayed by conventional MRI and DWI.DWI was better than conventional MRI in Group ??n=13?.DWI equaled conventional MRI in Group ??n=9?.Conventional MRI was better than DWI in Group ??n=18?.We recorded the onset time of each group,counted the ADC of lesion of each group and made a comparison with that of the other two groups respectively and compared ADC of the lesion and that of the corresponding parts of normal brain tissue.Results:The average onset time of Group ??n=13?in which DWI was better than conventional MRI was?3.3±1.6?days.The average onset time of Group ??n=9?in which DWI equaled conventional MRI was?6±2.7?days.The average onset time of Group ??n=18?in which conventional MRI is better than DWI was?13.1±5.8?days.The onset time differences among the groups had statistical significances.The ADC of three lesions and their corresponding parts of normal brain tissue were:?0.61±0.11?×10-3mm2/s and?0.83±0.08?×10-3mm2/s separately in Group ?,?0.86±0.07?×10-3mm2/s and?0.89±0.06?×10-3mm2/s separately in Group II and?1.18±0.21?×10-3mm2/s and?0.85±0.06?×10-3mm2/s separately in Group ?.The differences of the average ADC of Group I,Group ? and Group ? had statistical significances?P<0.05?.Compared with the ADC of the corresponding parts of normal brain tissue,the average ADC of Group ? decreased while the average ADC?P<0.05?of Group III increased and the average ADC?P>0.05?of Group ? equaled.Conclusion:DWI is of greater value than conventional MRI in early diagnosis of viral encephalitis and of early diagnosis value.Combining conventional MRI with DWI and ADC contributes to the judgement of the pathological stage of viral encephalitis.Part ? The Value of Conventional MRI and DWI to the Evaluation of the Prognosis of Viral EncephalitisPurpose:To analyze the value of conventional MRI and DWI to the evaluation of the prognosis of viral encephalitisMethods:With retrospective analysis of clinical and MRI characteristics for viral encephalitis in 47 cases,we found all of the 47 cases were with conventional MRI and DWI abnormalities.We should according to GOS divide them into two groups,eusemia and unfavorable prognoses and look for the relationship between conventional MRI,DWI and the prognosis of viral encephalitis through single factor analysis and multiple factor Logistic regression analysis.Results:In the 47 viral encephalitis patients with conventional MRI abnormalities,31.9%?15/47?got unfavorable prognoses.The single factor analysis showed that the multiple lesions shown on MRI,most lesions at level 2-3 and diffusion abnormalities had a close relationship with VE prognosis?P<0.05?.Multiple factor Logistic regression analysis showed that the multiple lesions?level 3?and diffusion abnormalities had a close relationship with VE prognosis.Conclusion:A wide range of multiple lesions shown on MRI is high risk factor for unfavorable prognoses of viral encephalitis.Diffusion abnormalities is high risk factor for unfavorable prognoses of viral encephalitis.
