| 【Objective】 This study introduces the application of MRI(Magnetic Resonance Image)and ROI(region of interest)and three-dimensional computed tomography imaging reconstruction to evaluate the efficacy and clinical research of the sclerotherapy based on the 26 patients on the head and neck of venous malformations(VMs).【Method】 In this study,our research team treated the 459 patients with peripheral venous malformations.There were 136 patients with peripheral venous malformations of head and neck from March in 2016 to October in 2012.Before we do the procedure of sclerotherapy,we classified the lesions according to MRI stigma and we have selected a sample of 26 patients with limited venous malformations.We divided all patients either treated once or twice in our clinic into two groups.It performed the percutaneous sclerotherapy by direct injection of group A Ethanol and group B Polidocanol foam sclerosants.Patients underwent MRI before and after sclerotherapy.We then imported MRI data into Mimics18.0 software.We have follow up on the patients 2-40 months after treatment.We selected the threshold ranges of venous malformations in ROI MRI.The regression rates of lesions were compared before and after sclerotherapy.According to the ROI-volumentry of MRI assessment of the three dimensional reconstruction of volume regression measurement and the response evaluation criteria in venous malformations as we used.The effects of VMs load percentage ranges in this study were divided into 4 levels,with Level I being poor(0%~25%)and level II being primary(26%~50%)and level III being as well(51%~75%)and level IV being excellent(76%~100%).Level III and Level IV percentage are considered as valid.【Result】 Group A had 16 out of 17 effective cases(94%)by RECIVM.Group B had 7 out of 9 effective cases(78%)by RECIVM.Efficient differences after treatment of volume between A and B group were not statistically significant(P value=0.776).Major complications after sclerotherapy are pain,fever,local swelling,etc.Ethanol group A had about one of 19 patients(6%)with local tissue ulceration and necrosis.The 26 patients were not present cardiopulmonary,cerebrovascular accident and other life-threatening complications.【 Conclusion 】 The method of ROI-volumentry of MRI assessment of three-dimensional reconstruction of volume regression measurement can reflect and evaluate the regression rates of the lesion defects in the percutaneous sclerotherapy of the 18 patients on the head and neck of limited VMs.Our experiences demonstrate the high potential for the adoption of this method.Treated the limited venous malformations have safety and small complications. |
