Influence To Blood Coagulation Function Of Taking Xuefu Zhuyu Decoction After Operation To Lower Limb Fracture

Objective : Through internal fixation for lower limb fracture,the patients were treated with Modified Xuefu Zhuyu Decoction was given after operation.The changes of blood coagulation function and the incidence of deep venous thrombosis were observed..Methods: From March 2018 to January 2019 in Jingdezhen Orthopedic Hospital of Traditional Chinese Medicine,72 patients with lower limb fractures who met the research criteria and underwent internal fixation,were randomly divided into treatment group(36 cases)and control group(36 cases).Within 12 hours and 1 week after operation,the treatment group was given oral Xuefu Zhuyu Decoction plus subcutaneous injection of low molecular weight heparin calcium injection,while the control group was only given subcutaneous injection of low molecular weight heparin calcium injection.Other routine treatments were the same.The coagulation function indexes and the incidence of deep venous thrombosis in lower extremities were detected and recorded respectively before operation,on the first day after operation,the third day after operation and the seventh day after operation.The changes of each examination result at different time points before and after treatment were compared between the two groups.Results: 1.There was no significant difference in preoperative prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),thrombin-antithrombin III complex(TAT)and D-Dimer between the two groups(P > 0.05).2.On the first day after operation,PT and APTT were shorter,FIB,TAT and D-Dimer were increased,and the difference was statistically significant(P < 0.05)compared with preoperative period.However,there was no significant difference between the two groups on the first day after operation(P > 0.05).3.On the third day and the seventh day after operation,compared with the first day after operation,PT and APTT gradually prolonged,FIB,TAT and D-Dimer gradually decreased,and the difference was statistically significant(P < 0.05).There was significant difference between the groups on the third day and the seventh day after operation and the first day after operation(P < 0.05).4.There was no deep venous thrombosis in lower extremity in the two groups before operation and on the first day after operation.Compared with the control group on the third and seventh day after operation,the incidence of deep venous thrombosis in the treatment group was lower than that of the control group,and the difference was statistically significant(P < 0.05).Conclusion: By comparing the combined use of the modified Xuefu Zhuyu Decoction and low molecular weight heparin calcium with the single use of low molecular weight heparin calcium for routine anticoagulation treatment after operation to lower limb fractures,we found that: 1)Modified Xuefu Zhuyu Decoction has more advantages in inhibiting coagulation factor activity,decreasing thrombin activity and fibrinogen production;2)It has a better clinical effect in improving slow blood flow,reducing blood hypercoagulability and reduce the risk of deep venous thrombosis occurrence in lower limbs.
