Objective:To investigate the differences of BDNF and various glycolipid metabolism indexes in type 2 diabetes group and normal control group.The effects of GLP1 receptor agonist(liraglutide)on BDNF and various glycolipid metabolism indexes in patients with type2 diabetes were further investigated.Methods:1.100 patients with Type 2 diabetes mellitus(T2DM),7.5% to 10.5%with diabetes mellitus(HbA1C)and 50 patients with non-diabetic healthy(C)were selected as control.The age and sex of the selected population were matched.2.Height,weight,waist circumference,hip circumference,waist to hip ratio,glycosylated hemoglobin,fasting blood glucose,postmeal blood glucose,adiponectin,leptin,BDNF levels and tyrosine kinase receptor B levels were measured.3.After 8 weeks of lifestyle intervention,diabetic patients were randomly divided into two groups: one group was treated with metformin(M),the initial dose of metformin was1500mg/ days,adjusted according to blood glucose and glycosylation level,The maximum dose is 2000mg/ days.In the other group,the initial dose of Lilaglutide in the treatment of(L),was 0.6mg/ days,adjusted according to the level of blood glucose and glycosylated hemoglobin,the maximum dose was 1.8mg/ days,and the above indexes were redetermined after 3 months.The changes of BDNF and other indexes were observed.4.The software SPSS22.0 was used to process all the data,and the linear correlation method was used to analyze the correlation between BDNF level and glucose metabolism index.Results:1.The levels of BMI,HbA1 c and 2hPG in group M and group L were significantly higher than those in C group、WC、WHR、HbA1c、FINS、2hPG、and FINS(P < 0.01).2.BMI,WC,HC,WHR,HbA1 c,FPG,2hPG,IRI in group M was significantly lower than that before intervention(P < 0.01).BMI,WC,HC,WHR,HbA1 c,FPG,2hPG,IRI in group L was significantly lower than that before intervention(P < 0.01).There was no significant difference between group M and group L in BMI、 WC、 HC、 WHR、HbA1c、FPG、2hPG、IRI group(P > 0.01).3.the level of BDNF in group M and group L was significantly lower than that in group C before intervention;and the level of BDNF in group L was significantly higher than that before intervention(t = 15.965,P < 0.01),There was no significant change in the level of BDNF in group M(t = 1.658,P = 0.104).4.Compared with group C,there was no significant change in TrkB level in group M and group L before intervention.Compared with before intervention,the level of TrkB in group L was significantly higher than that before intervention(t = 4.078,P < 0.01).The level of group M was higher than that of group M(t = 3.81,P < 0.01).5.Compared with group C,the level of LP in group M and group L was significantly higher than that in group C before intervention.Compared with before intervention,the level of LP in group M was significantly lower than that before intervention(t = 2.724,P= 0.009).There was no significant change in L group(t=1.172,P=0.247).6.Compared with group C,there was no significant change in ADP level in group M and group L before intervention.Compared with before intervention,the level of ADP in group L was significantly higher than that before intervention(t = 5.67,P < 0.01).The level of group M was not significantly higher than that before intervention(t = 17.632,P= 0.104).7.Univariate correlation analysis showed that the level of BDNF in human serum was positively correlated with TrkB(r = 0.411,P < 0.01),ADP(r =0.213,P < 0.01).There was no significant correlation in the index of residual glucose metabolism(P >0.01).Conclusion:The level of brain-derived neural factors(BDNF)in group M and group L was significantly lower than that in group C.The content of BDNF in group L was significantly higher than that before intervention,but there was no significant change in group M.It is suggested that BDNF may be involved in the pathogenesis of diabetes and play a certain role in the occurrence and development of T2 DM,while Lilaglutide can improve blood glucose control by increasing the content of BDNF.
