Objective:To explore weather total laparoscopic esophagogastric devascularization with splenectomy for portal hypertension is safety and effectiveness or not. Methods:CNKI,Wan Fang Data Knowledge Service Platfo,VIP,PubMed,Cochrane Library,Science Direct were used to search the literature on laparoscopic surgery and open surgery for portal hypertension. We use RevMan 5.3 for Meta analysis. Results:A total of 9 articles were included involving 901 patients,473 LS and 428 OS.Meta analysis showed that LS was superior to OS in blood loss(WMD=-168.81 ml,P<0.00001),postoperative hospital stay(WMD=-2.33 d,P=0.008)and postoperative complications(RR=0.47,P<0.00001),and there was no statistical significance between them in operative time(WMD=30.45 min,P=0.17)and hospitalization costs(WMD=-0.22 K,P=0.67).Conclusion:Compared with open surgery, laparoscopic and open esophagogastric devasculariz ation with splenectomy have the andantages of less blood loss, shorter postoperative hospital stay and less postoperative complications. It should be recommended in larg e hepatobiliary surgery center.
