Objective:This study aims to observe the relationship between the proximal urethra and the bladder neck of postpartum women,and the morphological changes of the pelvic fissures,to evaluate the therapeutic effect of pelvic floor rehabilitation in postpartum women by perineal three-dimensional ultrasound.Methods:From December 2017 to December 2018,patients thoes who in the postpartum review from the Obstetrics and Gynecology Department of Shanxi Dayi Hospital,18 to35 years old,single-term full-term delivery,body mass index(BMI)<29 kg/m~2,pelvic floor muscle strength<IV,neonatal weight between 2500 and 4000g,and informed consent were selected.30 patients who received standard pelvic floor rehabilitation treatment were selected as the study group,while 30 cases excluded from pelvic floor rehabilitation treatment as control group.Three dimensional reconstruction technology was used to measure the morphological changes of the pelvic diaphragm in two states of rest and Valsalva,including the characteristic parameters of bladder neck descent(BND),retrovesical angle(RVA),urethal rotation angle(URA),urethral mouth funnel formation,anterior and posterior diameter(LH-P),transverse diameter(LH-T),circumference (LH-L),as well as area(LH-A).And measure the same data again 12~14 weeks after delivery.All the data were statistically analyzed.Results:1.Three-dimensional ultrasound study of perineal patients enrolled patients 6~8weeks postpartum,the differences between study and controll group in BND,RVA,URA,urethral mouth funnel formation and LH-P,LH-T,LH-L,LH-A had no statistical significance(P>0.05).2.Three-dimensional ultrasound study of perineal patients enrolled patients 12~14weeks postpartum,the differences between study and controll group in BND,RVA,URA and urethral mouth funnel formation were statistcally significant(P<0.05).3.Three-dimensional ultrasound study of perineal patients enrolled patients 12~14weeks postpartum,the differences between study and controll group in LH-P,LH-T,LH-L,LH-A were not statistcally significant,and under the condition of Valsalva,LH-T,LH-L,LH-A increased and had statistical significance in the study group compared with the control group(P<0.05),while LH-P had no statistical significance(P>0.05).4.In the study group,before and after treatment,the parameters in resting state and maximum Valsalva state were significantly different(P<0.05).Conclusion:1.As a kind of pelvic floor ultrasound diagnosis technique,the perineal three-dimensional ultrasonography is an important auxiliary examination for the comprehensive and objective observation of the morphological changes of the anterior pelvic cavity and pelvic orifice of postpartum women.2.Three-dimensional pelvic floor ultrasound via perineal provides early thinking,early treatment and early control of pelvic floor disease development.It has important clinical application value in evaluating postpartum female pelvic floor rehabilitation treatment.
