Objective:To explore and analyze the clinical application value of oral administration of different concentrations of iodine preparation combined with intramuscular injection of anisodamine(654-2)in the diagnosis of gastric malignant tumor by multi-slice spiral CT.Methods:192 patients with gastric malignant tumors who underwent multi-slice spiral CT plain scan and enhanced scan from September 2016 to December 2018 in our hospital were collected. All patients were confirmed by pathology as gastric malignant tumors. The patients were randomly divided into six groups,32 in each group.Among them, patients in group I-III were given oral iodine preparations(0,555 mg/ml iodine preparation,2775 mg/ml iodine preparation)for gastrointestinal filling preparation without anisodamine(654-2),IV-VI group patients with different concentrations of iodine preparation(0,555mg/ml iodine preparation,2775mg/ml iodine preparation)for gastrointestinal filling preparation and intramuscular injection of anisodamine(654-2).Then,a multi-slice spiral CT was used for plain and enhanced examination. Two imaging physicians with the title of director or deputy chief physician and a senior physician with gastrointestinal tumor surgery jointly diagnosed the images. The degree of gastric cavity filling and lesion invasion, density, degree of enhancement, and thickness of the stomach wall were evaluated. The diagnostic expert group diagnosed and statistically analyzed six groups of patients based on these evaluation indicators. Results:There were no obvious peristaltic artifacts in CT images obtained from 192 patients with gastric malignant tumors after multi-slice spiral CT plain and enhanced examination. The excellent rate of gastric filling in group V was 100.0%(32/32).The diagnostic accuracy of group ? in group ?,II and III was 81.3%(26/32),and the accuracy of group V in group IV,V and VI was higher in group V(93.8%(30/32),I-VI).The diagnostic accuracy of group V was the highest in the group, and the diagnostic accuracy of each group in I-VI group was P>0.05.The difference was not statistically significant. The total diagnostic accuracy of the ?-? group without intramuscular injection of anisodamine was compared with the ?-? group of intramuscular injection of anisodamine,P>0.05,and the difference was not statistically significant. The incidence of gastric wall thickness in the lesion stomach wall, adjacent stomach wall and non-lesional stomach wall of group IV,V and VI was slightly lower than that of group ?,? and ?.120 cases of gastric cancer mainly invaded the gastric antrum(58.3%),42 cases of gastric stromal tumor mainly invaded the stomach(accounting for 71.4%),and 30 cases of gastric lymphoma mainly caused by multiple parts(73.3%).Most of the gastric cancer showed uniform and obvious enhancement, and gastric stromal tumors showed obvious uneven enhancement. The gastric lymphoma showed more uniform and mild enhancement. Conclusion:In the case of patient's condition, oral administration of 555mg/ml iodine preparation combined with intramuscular injection of anisodamine before multi-slice spiral CT examination can improve the diagnostic accuracy of gastric malignant tumors.Multi-slice spiral CT findings of different types of gastric malignant tumors have their own characteristics.Multi-slice spiral CT have important value in the diagnosis and differential diagnosis of gastric malignant tumors.
