Objective: To study the expression levels of serum neuropeptide y(NPY),serum and intestinal mucosal peptide yy(PYY)in patients with diarrhea-predominant irritable bowel syndrome(IBS-D);to evaluate the anxiety and depression state of IBS-D patients;to explore the relationship between the expression levels of NPY and PYY and anxiety and depression state in IBS-D patients,so as to provide new information for the diagnosis and treatment of IBS-D.Methods: Twenty-eight IBS-D patients in the Department of digestive medicine of Hebei People's Hospital were selected as the study subjects and 20 healthy people as the control group.Self-rating Anxiety Scale(SAS)score and Self-rating Depression Scale(SDS)score were performed.Fasting serum was collected and serum NPY and PYY levels were measured by Elisa method.The expression of PYY in ileum and colon mucosa was detected by immunohistochemistry.Results:1.The serum NPY concentration of IBS-D group was lower than that of healthy control group(t=-3.023,P<0.05).2.The serum PYY concentration,the average optical density of PYY in the ileum mucosa and the average optical density of PYY in the colonic mucosa of the IBS-D group were higher than those of the healthy control group(t=2.319,P<0.05),(t=9.479,P<0.05),(t=9.337,P<0.05).The average optical density of PYY in ileum and colonic mucosa of IBS-D group was positively correlated with serum PYY concentration(r=0.632,P<0.05),(r=0.548,P<0.05).3.The SAS score and SDS score of IBS-D group were higher than those of healthy control group(t=2.715,P<0.05),(t=4.838,P<0.05).The IBS-D group had a positive correlation between SAS score and SDS score(r=0.609,P<0.05).4.Serum NPY concentration in IBS-D group was negatively correlated with SAS score and SDS score(r=-0.707,P<0.05),(r=-0.536,P<0.05).5.There was no significant correlation between serum PYY concentration,ileum and colonic mucosal PYY mean optical density and SAS scores in IBS-D group(r=-0.096,P>0.05),(r=-0.293,P>0.05),(r=-0.128,P>0.05);There was no significant correlation between serum PYY concentration,ileum and colonic mucosal PYY mean optical density values and SDS scores in the IBS-D group(r=-0.064,P>0.05),(r=-0.182,P>0.05),(r=-0.187,P>0.05).Conclusions:1.IBS-D patients had low serum NPY expression,high anxiety and depression scores,and serum NPY expression levels were negatively correlated with anxiety and depression scores.Abnormal NPY expression levels and anxiety and depression may be involved in the pathogenesis of IBS-D.2.The expression of PYY in serum,ileum and colonic mucosa of IBS-D patients is high,and the expression level is the same.The abnormal expression level of PYY may participate in the pathogenesis of IBS-D.Clinically,the expression level of PYY in intestinal mucosa can be indirectly reflected by measuring the concentration of PYY in serum.
