A Preliminary Study On The Mechanism And The Intervention Effect Of Mindfulness-Based Music Therapy On Postpartum Depression

Objective : Postpartum depression mainly manifested as emotional decline,decreased concentration,sleep disorders,even having serious suicidal tendencies or suicide plans,which had a lot of negative effects on the mother.The incidence of postpartum depression is 9.2-15% in China.The commonly used treatments for postpartum depression are antidepressants,and psychotherapy.However,the current intervention measures need too high professional requirements,were not suitable for a wide range of popularize,or its intervention mode is not suitable for the special status of pregnant women and.So this study tries to find a more convenient and effective psychotherapy for postpartum depression women,prove its effectiveness and explore the mechanism of its possible action.Methods: In this study,we searched the literature on mindfulness-based therapy for postpartum depression in Pubmed,Web of Science,Cochrane Library Plus,Embase,Cinahl,Psycinfo,CNKI,WanFang,WeiPu and other domestic and international databases.The statistical analysis is carried out with RevMan software.Referring to Meta-analysis results,and the domestic and foreign literature,we used convenient sampling method to select postpartum depression mothers through a third-class hospital in Shijiazhuang,then we used mindfulness music therapythrough network video to intervene them for eight weeks,and observe the effect of the therapy for postpartum depression(EPDS),maternal self-efficacy(GSES),mindfulness level(MAAS),sleep quality(PSQI).Based on the results of maternal intervention,we used a convenient sampling method to select 50 volunteers with depression,using group mindfulness music for eight weeks of intervention.The effect of psychological intervention on depression(BDI),self-efficacy(GSES)was observed before and after 8 weeks.At the same time,the EEG data of the depressed volunteers at rest and listening to music were recorded before and after 8 weeks,and their relaxation and concentration were calculated.The data were processed and analyzed by spss 20.0 software.Results: 1 Finally,four articles were included in the analysis.the literature quality evaluation was moderate,and there was high heterogeneity among the various studies,so the random effect model was used to analyze the results.The results showed that the depression score of the intervention group based on psychotherapy was significantly lower than that of the control group(SMD =-3.37,95% CI-5.8 ~-0.88,P < 0.01).The results show that psychotherapy based on mindfulness can improve postpartum depression.2 Intervention effect of mindfulness music on maternal women 2.1 Effect of mindfulness Music on Maternal EPDS,PSQI,MAAS,GSESThere was interaction between the group factor and the time factor of the EPDS,PSQI,MAAS,GSES score of puerpera(P < 0.05).The simple effect test showed that,fixed the time factor,there was no significant difference between the two groups at baseline.After 8 weeks,the EPDS score of the intervention group was significantly lower than that of the control group,and the MAAS,GSES score of the intervention group was significantly higher than that of the control group;fixed the group factor,the EPDS,PSQI score of intervention group was significantly lower than that of pre-intervention score,and the MAAS,GSES score of intervention group was significantly higher than that of pre-intervention score(P < 0.05),while there was no significant change in control group(P > 0.05).The difference of EPDS,MAAS,GSES score between the two groups was statistically different(P<0.05).2.2 Analysis of the effect of three-month follow-up in intervention groupAfter 8 weeks,the EPDS,PSQI,MAAS,GSES scores of the intervention group were matched with those of the three-month follow-up.The results showed that there was no statistical difference among the three scales(p > 0.05).There was a correlation between the changes of the MAAS scale with the EPDS scale,the GSES scale in baseline and after 8 weeks.2.3 Correlation analysis of absolute difference value of each scale before and after interventionPearson correlation analysis showed that the greater the absolute value of maas difference before and after intervention in the intervention group,the greater the absolute value of EPDS,GSES difference,and the correlation was statistically significant(r = 0.44,P < 0.05;r = 0.535,P < 0.05).The greater the absolute value of MAAS difference,the smaller the absolute value of PSQI difference was,but the trend was not statistically significant(r =-0.124,P> 0.05).These results suggest that mindfulness music has a good effect on maternal depression and improves the quality of sleep,self-efficacy and mindfulness,and this effect can last until three months later.After the intervention of mindfulness music,the increase of mindfulness level was positively correlated with the improvement of depression and self-efficacy.3 Intervention effect of mindfulness Music on volunteers 3.1 Effect of mindfulness Music on BDI,GSES in volunteersThere was interaction between group factor and time factor in BDI,GSES score of volunteers(P < 0.05).The simple effect test showed that,fix the time factor,there was no significant difference between the two groups at baseline.After 8 weeks,the BDI score of intervention group was significantly lower than that of control group(P<0.05);fix the time factor,the scores of BDI after intervention in intervention group were significantly lower than those before intervention(P<0.05),GSES score in intervention group was significantly higher than that before intervention(P<0.05),but there was no significant change in control group(P<0.05).3.2 Effect of mindfulness music on concentration and relaxation of volunteers Concentration and relaxation were calculated by EEG data.Repeated ANOVA was used to measure the concentration in both groups,and there was interaction between the grouping factor and the time factor(p < 0.05),The simple effect test showed that,fix the group factor,after 8 weeks,the concentration of resting and musical state in the intervention group was significantly higher than that before intervention(p < 0.05),but there was no significant change in the control group(p > 0.05).The degree of relaxation at baseline was higher than that under music(P<0.05),but there was no significant difference between the rest and the music after eight weeks in the intervention group.3.3 The correlation between the degree of concentration and relaxationAt baseline,there was a negative correlation between concentration and relaxation;after 8 weeks,there was no correlation between the two groups.3.4 Correlation between depression and concentrationThere was a negative correlation between the improvement of mood before and after 8 weeks and the improvement of concentration in music state(r =-0.449,P < 0.05).The results showed that mindfulness music therapy could not only improve depression,self-efficacy,but also improve the concentration and relaxation of depressive patients.The improvement of concentration and relaxation may play an intermediary role in the improvement of depression,which may be one of the mechanisms of the therapy to improve depression.Conclusion: The mindfulness-based music therapy can effectively improve postpartum depression and sleep,improving the level of mindfulness and self-efficacy of mothers.Moreover,the combination of mindfulness and music relaxation can improve the students' concentration and relaxation,and further improve their depression and self-efficacy.In the future study,we can take the parturients as the research object,and discuss the mechanism of the intervention effect of mindfulness music on this special group.
