Fecal Incontinence(Fecal Incontinence,FI)is a common clinical symptoms,the patient's sphincter can not control the discharge of feces and gas,feces outflowautomatically from the anus.Although the disease is not a direct threat to life,but cause the patient's physical and mental suffering,interfere with the normal life and workseriously.With the development of medicine and engineering,people put forward the method of artificial anal sphincter treatment.This method can provide a complete solution to the problem of Fecal Incontinence.However,the existing artificial anal sphincter can not establish the function of defecation,can not control the defecation time.It needs to control the prosthesis by repeated extrusion,and the operation is complex and expensive.Aimed at more common fecal incontinence,combined with the skin supply and wireless communication control technology.Study on implantable artificial sphincter system combined bowel control and detection and more in line with the physiological requirements of the human body has become an important direction for the cross study of Medicine and Engineering.Based on the National Natural Science Foundation of China(NO.31170968)and the Shanghai committee of science and technology research projects(NO.14441902800?NO.15441903100),combined with years of research results,this paper studies the key technologies of the feedback artificial anal sphincter system based on the skin for the artificial anal sphincter system,carrying out the imitation of pubic rectum muscle type artificial anal sphincter system research.The system has been optimized in many aspects,so that the system can achieve a great improvement in the control effect,operation time,low power consumption,perception sense and so on.The main research work in this paper is mainly embodied in the following aspects:1.Developing of bionic sphincter of imitation of the puborectalis muscle actuators and adopts DC micro motors and micro gear reducer drive executive mechanism,through theory and experiment to optimize the structure and material,exploring driven unit parameters,researching adaptability and blood safety of imitation puborectalis muscle implementation structure when works in the human body environment.2.According to the imitation of the puborectalis muscle implementation structure carried out the research of biological pressure sensing module applying this structure,including the principle of sensor design,package design,the temperature and pressure calibration test and stability test.5 pressure sensor modules are embedded inside the actuator to detect different position of intestinal pressure information;through animal experiment in vivo in vitro,extracting related information so as to establish the defecation alarm and the corresponding relationship between threshold pressure.3.Optimization design of hardware and software of the control system in vivo and in vitro,focus on the system of low power consumption.Introducing the analog switch on hardware circuit to control the power of the sensor module,software processing is: system working mode can be divided into intermittent cycle detection mode and continuous operation mode,system works normally in the intermittent cycle detection mode,greatly reduce power consumption,power consumption reduced by 90% after processing.4.In this paper,the system integration and encapsulate the in vivo and in vitro,to integrate the system in the body,including the control unit,execution unit,energy receiver coil.Integrated circuit board and batteries using ABS shell encapsulation,and sealed with medical silica gel.Sphincter receiving coil actuator and energy through sealed lead connected to the circuit board.Integrated design makes the system more compact structure,improve the biocompatibility of the system at the same time convenient implant surgery.5.System completed in vitro animal experiment and laboratory animals experiment,and in vitro experiments using pig intestine,in vitro experiments verify the effectiveness and stability of the system.In vivo animal as the beagle,experiment plan for 3 months,the animals spent rejection phase successfully and gradually restored to health.The experiment on the imitation of puborectalis muscle type of artificial anal sphincter system control effect,wireless charging energy,communications and biocompatibility in vivo and in vitro are verified,at the same time through the pressure acquisition module,wireless communication module,host desktop module record the intestinal pressure information,obtained abundant experimental data and experience,provided the basis for establishing correct relationship between intestinal waste quantity and the pressure value.
