Objective:Diabetes mellitus is a clinical syndrome caused by genetic and environmental factors.It is a worldwide public health problem that seriously threatens human health.Diabetic retinopathy,a chronic complication of diabetes,is one of the main causes of blindness.In this study,we observed the expression of serum fibroblast growth factor 19(FGF19)and pigment epithelium derived factor(PEDF)in patients with type 2 diabetic retinopathy(DR),analyzed their correlation with DR,and explored their role in the occurrence and development of diabetic retinopathy.Methods:89 patients with type 2 diabetes who were hospitalized or outpatient in Baoding First Central Hospital from September 2017 to December 2018 were selected as the experimental group,including 44 males and 45 females,aged 55.75 + 7.26 years.At the same time,40 health examinees in the physical examination department of our hospital were randomly collected as the healthy control group(NDM group).General data of all subjects were collected,such as age,height,weight,blood pressure,and clinical indicators: fasting venous blood glucose(FBG),serum insulin(FINS),cholesterol(TC),triglyceride(TG),low density lipoprotein(HDL),high density lipoprotein(HDL),glycated hemoglobin(HbA1c),homocysteine(HCY),C-reactive protein(CRP).Serum levels of ALT,AST,Scr and serum levels of fibroblast growth factor 19 and PEDF were measured.According to diabetic retinopathy,type 2 diabetes mellitus was divided into two groups: 45 patients with type 2 diabetes mellitus alone(DM group)and 44 patients with type 2 diabetes mellitus combined with retinopathy(DR group).SPSS 19.0 statistical software was used for statistical analysis,and the measurement data of normal distribution were expressed as mean ± standard deviation.Single-factor analysis of variance was used for multigroup comparison,LSD was used for two-way comparison,Pearson grade correlation analysis was used for correlation analysis,and ROC was used to analyze the diagnostic efficacy of PEDF and FGF19 for DR,and to evaluate the value of serum levels of FGF19 and PEDF in predicting the occurrence and progression of DR.P < 0.05 was statistically significant.Results:1.The serum FGF19 level in the DR group was lower than that in the DM and NDM groups(P<0.05).The serum PEDF level in the DR group was higher than that in the DM and NDM groups(P<0.05).2.The FBG of DM and DR group was higher than that of NDM group(P<0.05),but there was no significant difference between DM group and DR group(P>0.05).The difference of HCY level in DR group was higher than that in DM and NDM group(P< 0.05),but there was no significant difference between DM group and NDM group(P>0.05).The levels of FINS,HbAlc,TG,TC,CRP and LDL in DR group were higher than those in DM and NDM groups(P<0.05).The DM group was higher than the NDM group(P<0.05),and the difference was statistically significant.The HDL level in the DR group was lower than that in the DM and NDM groups(P<0.05),and the DM group was lower than the NDM group(P<0.05).The differences were statistically significant.3.Pearson grade correlation analysis showed that serum FGF19 level was negatively correlated with CRP in patients with type 2 diabetic retinopathy(r=-0.319,P<0.05),and negatively correlated with TC(r=-0.301,P<0.05).There was no significant correlation between PEDF in DR group and each index.The P value was greater than 0.05,which was not statistically significant.4.The area under the ROC curve of FGF19(AUC)was 0.978(P<0.05);the AUC of PEDF was 0.796(P<0.05).The best diagnostic cutoff value for calculating FGF19 is 7.97,and the best diagnostic cutoff value for PEDF is 7.81.Conclusions:There is a correlation between type 2 diabetic retinopathy and FGF19 and PEDF.The serum FGF19 and PEDF are elevated in patients with type 2 diabetic retinopathy.FGF19 and PEDF can be predictors of DR,and the diagnostic value of DR has certain significance.
