ObjectiveTo compare the risk of 1-year ischemic stroke recurrence among patient with atrial fibrillation detected after stoke?known atrial fibrillation before stroke and sinus rhythm and investigate whether insular ininfration are different among three groups.MethodsThis study is based on a retrospective cohort study.We recruited 222 acute ischemic stroke patients in the department of neurology,the First Affiliated Hospital of Chongqing Medical University from 2015.10-2017.6.According to heart rhythm collected on admission,we divided patients into three groups: atrial fibrillation diagnosed after stoke,known atrial fibrillation before stroke and sinus rhythm.Multivariate logistic regression analysis was used to compare the 1-year recurrence rate between the three groups and the prevalence of insular strokes among the 3 groups.ResultsOf the 222 patients with ischemic stroke,28 had atrial fibrillation diagnosed after stroke,54 had known atrial fibrillation before stroke,and140 had sinus rhythm.6 patients(21.43%,n = 28)with new atrial fibrillation diagnosed after stroke,9 patients with known atrial fibrillation(16.67%,n = 54),and 7 patients with sinus rhythm(5%,n = 140)had recurrent ischemic stroke occurred within one year(p = 0.005).After adjusting for the relevant risk factors,the recurrence rate of ischemic stroke within one year after stroke in the new atrial fibrillation diagnosed after stroke group and the known atrial fibrillation group was higher than that of sinus rhythm(OR = 6.702,95% CI = 1.231-36.477;OR = 5.678,95% CI =1.021-31.567),the one-year stroke recurrence rate in the new atrial fibrillation diagnosed after stroke group was not significantly different from that in the known atrial affilation group.The incidence of insular infarction in the new atrial fibrillation diagnosed after stroke group was higher than that in the sinus rhythm group(32.14% vs 10,71%,p <0.05).ConclusionsThere was no significant difference in the recurrence rate of stroke in AFDAS compared with KAF;insular infarction was associated with the occurrence of AFDAS.
