Early Efficacy Of Da Vinci Surgical Robot Assisted Laparoscopic Sleeve Gastrectomy In The Treatment Of Obesity

Objective: To explore the feasibility and effectiveness of Da Vinci Surgical Robot assisted laparoscopic sleeve gastrectomy(RALSG)for the treatment of obesity and obesity-related diseases.Methods: Clinical and follow-up data of 41 cases of RALSG performed in Tongji Medical College affiliated Union Hospital,Huazhong University of Science and Technology between March 2015 and May 2018 were retrospectively analyzed.Statistical methods were used to analyze and to compare statistical differences of preoperative and post-operative body weight,blood sugar,blood lipid related indicators and obesity-related diseases.Results: All patients underwent RALSG without conversion to laparotomy or other surgical procedures.The average operation time was(127.1±19.4)minutes.The average amount of bleeding during operation was(24.3±8.6)ml.The average hospitalization time after operation was(4.7±2.5)days.One case had gastric stump fistula,one case had intermuscular venous thrombosis of lower extremities,and 7 cases had new gastroesophageal reflux disease.There is no death,digestive tract obstruction,upper gastrointestinal bleeding,abdominal bleeding,wound infection and other complications.No patient was lost or died within post-operative follow-up visit.Body weight,blood sugar,blood lipid related indicators were significantly improved after post-operative 6 months(P<0.05).Compared with preoperative body weight related indicators,body weight fell to(98.3±19.5)kg(P<0.05),BMI fell to(33.7±5.0)kg/m2(P < 0.05),EWL% rose to(39.9 ± 12.8)%(P < 0.05).Compared with preoperative blood sugar related indicators,FPG dropped to(4.4±0.5)mmol/L(P<0.05),PBG dropped to(9.1±0.9)mmol/L(P<0.05),HbAlc dropped to(5.3±0.6)%(P<0.05).Compared with preoperative blood lipid related indicators,TG dropped to(1.6±0.7)mmol/L(P<0.05),TC dropped to(3.4±0.6)mmol/L(P<0.05),LDL dropped to(1.8±0.4)mmol/L(P<0.05),HDL rose to(1.8±0.3)mmol/L(P<0.05).All obesity-related diseases were improved and partly cured six months after operation.Conclusion: Da Vinci Surgical Robot assisted laparoscopic sleeve gastrectomy(RALSG)is a safe and feasible method for the treatment of obesity and obesity-related diseases.
