Object:The present study was designed to investigate the effects of Shenmai injection on cardiac output?cardiac output,CO?,stroke output?stroke volume,SV?and left ventricular ejection fraction?ejection fraction,?in patients with off-pump coronary artery bypass grafting?Off-pump coronary artery bypass graft,OPCABG?on protective effect of Shenmai injection on cardiac function in patients with OPCABG.Methods:A total of 40 patients scheduled for OPCABG under general anesthesia,including age 4560 years,body mass index?body mass index,BMI??<25?kg/m2,ASA?American Association of anesthesiologists?grade IIIII grade,and without organ dysfunction were enrolled from October 2016 to July 2017.The patients were randomly divided into SM group?n=20?and the 5%glucose?G?group?n=20?.After induction of anesthesia,Shenmai injection was diluted to 150 ml with 5%glucose in SM group,and150ml/h was injected through the central vein at the beginning of the operation,while 5%glucose injection was injected into the central vein at the same rate in G group.After the entrence of patients,patients were subjected to radial artery puncture and right internal jugular vein puncture under local anesthesia.Before induction,high-flow oxygen inhalation was given for 3 minute.Sufentanil 1ug/kg,midazolam 0.05mg/kg,lidocaine3ml,etomidate 1-2 mg/kg and cis-atracurium 0.1 mg/kg/kg injected intravenously in turn during induction.After 5min,the endotracheal tube was inserted by visual laryngoscope.The respiratory parameters of anesthesia machine were set as follows:?VCV?,oxygen flow 2L/min,tidal volume?VT?6-8ml/kg,respiratory rate?RR?12-14bmp,PETCO2 3545 mm/Hg.Maintain the depth of anesthesia:Intraoperative pump propofol 4mg/kg/h and remifentanil 0.2 ug/kg/h while inhaling of 1%2%sevoflurane and using sufentanil and cis-atracurium intermittently to maintain appropriate depth of anesthesia,BIS 40/60.Nasopharynx temperature,anal temperature and urine volume of patients were monitored.Vasoactive drugs such as dopamine,nitroglycerin and methoxyamine were using to regulate blood pressure.EF was monitored by transesophageal ultrasound during operation,and CO,SV was continuously monitored by?Flo Trac/Vigileo?Flo Trac sensor of Vigileo cardiac output monitor.The values of CO,SV,and EF before induction?t1?,at the beginning of operation?t2?,30 minutes after the start of operation?t3?,at the beginning of coronary artery bypass graft?t4?,at the end of coronary artery bypass graft?CABG??t5?,and at the end of operation?t6?were recorded.Results:The values of CO,SV,and EF in the patients of SM group at t3 to t6 were found to be significantly higher than those at t1?P<0.05?.The values of CO,SV,and EF in the patients of G group were found to be increased at t5 and t6?P<0.05?.At t3 and t4,the values of CO,SV,and EF in SM group were significantly higher than those in the G group?P<0.05?.Conclusion:In patients with OPCABG under general anesthesia,Shenmai injection can effectively improve the cardiac function and effectively increase the values of CO,SV,and EF during operation.Significance:Shenmai injection has protective effect on cardiac function in patients undergoing OPCABG and improves the safety of anesthesia management in the perioperative.
