Objective:Initially we test the serum homocysteine and hypersensitive C-reactive protein in the patients who have dysphagia(Meridians and collaterals)of different TCM syndrome types after experiencing the stroke.We also perform the water swallow test on the patients of different TCM syndrome types,assess the damage level of patient s swallowing function,and study the level difference of serum homocysteine and hypersensitive C-reactive protein in different TCM syndrome types,the correlation between the swallowing function damage degree of different TCM syndrome types and the two index levels.Furthermore,we expolore the medical basis for the circulatory symptoms of the TCM syndrome differentiation of dysphagia(Meridians and collaterals),as well as the connection between the serum homocysteine and hypersensitive C-reactive protein and the TCM syndrome types of dysphagia(Meridians and collaterals)after a strok.Mothod:Clinically we collect 120 cases which meets the requirement,which include 90 inpatients who have dysphagia(meridians and collaterals)after stroke,and 30 physicla examiners.Based on Criteria for Diagnosis and Evaluation of Therapeutic Effect of Stroke as well as Criteria for Diagnostic and Therapeutic Effects of TCM Diseases in Traditional Chinese Medicine Industry,the patients with the post-stroke dysphagia(Meridians and collaterals)are categorized into the three tyes of wind and phlegmobstructing collaterals,liver and kidney deficiency,Qi deficiency and blood stasis respectively.The patients have fasting extraction of elbow venous blood in the next morning after admission while the physical examiners have fasting extraction of blood the next morning.This is to detect the value of serum homocysteine and hypersensitive C-reactive protein and analyze the two indexes.On the day of entering the hospital,the patients with post-stroke dysphagia have the clinical water swallow test for the purpose of evaluating the critical degree of dysphagia in different syndrome types.A statistical analysis is made with the connection of all the TCM syndrome types,the result of water swallowing test,serum homocysteine and hypersensitive C-reactive protein.Result:The two index levels of serum Hcy and hs-CRP in the patients with dysphagia(Meridians and collaterals)after stroke are significantly higher than the the normal control group,P<0.05;there are obivious differences in the levels of Hcy and hs-CRP among different syndrome types,P<0.05;from low to high,the levels of Hcy and hs-CRP are wind and phlegm obstructing collaterals,Qi deficiency and blood stasis,liver and kidney deficiency.There is difference among the damage degree of the swallowing functions of the patients with the three different TCM syndrome types,P<0.05;the patients of wind and phlegm obstructing collaterals mainly have mild degree and take up 61.8%;the patients of Qi deficiency and blood stasis have a moderate degree of damage,accounting for 48.4%;the patients with liver and kidney deficiency show the severe degree,accounting for 52%.There are high differences in the levels of serum Hcy and hs-CRP and the damage degree of the swallowing function in various syndrome types,P<0.05;the levels of serum Hcy and hs-CRP increase correspondingly as the severe degree of swallowing function damage becomes worse.In the cases studied in the subject,the traditional risk factors such as hyperlipidemia,hypertension and diabetes don t influence serum Hcy and hs-CRP of all the TCM syndrome types while the increasing levels of serum Hcy and hs-CRP are independent danger factors.Conclusion: The levels of serum Hcy and hs-CRP in the patients with the dysphagia(Meridians and collaterals)after having a stroke are correlated with all the TCM syndrome types.In all the syndrome types,the correlation of the levels of serum Hcy and hs-CRP reflects as: liver and kidney deficiency comes as the highest,Qi deficiency and blood stasis ranks the secondary place,and wind and phlegm obstructing collaterals is the lowest.The research result indicates that the levels of serum Hcy and hs-CRP can be regarded as the objective indexes for the TCM syndrome differentiation and categorization of the patients with with dysphagia after stroke.There are also differences in the dysphagia degree of different patients with all the TCM syndrome types.Based on the dysphagia degree from high to low,the three types are liver and kidney deficiency,Qi deficiency and blood stasis,and wind and phlegm obstructing collaterals respectively.The levels of serum Hcy and hs-CRP are correlated with the patients dysphagia.It s found that the more serious the patients dysphagia damage becomes after the stroke,the higher the levels of the serum Hcy and hs-CRP will become.The study result also indicates that the increasing levels of the serum Hcy and hs-CRP are the risk factors of the patients with dysphagia after stroke.
