| [Objective]:Associate Professor Huang has his unique knowledge and rich experience in the differentiation,prognosis and nursing of pediatric pulmonary diseases.However,his current academic thoughts and clinical experience of pediatric pulmonary diseases have not been completely and systematically compiled.After studied the academic thoughts of Associate Professor Huang,the research summarized the academic thoughts of the tutor on pediatric lung diseases and the experience of some pediatric pulmonary diseases.I hope that through this article,I will sort out the academic thoughts and clinical experience of Associate Professor Huang,and get improved in the future.[Methods]:1.Establish a basic framework for pediatric pediatric lung diseases,after the study of "Traditional Chinese Medicine Pediatrics",internal textbook of QiHuang College of Jiangxi University of TCM " Traditional Chinese Medicine Dialectical Treatment Program",and search of related papers on the HowNet;and accordingly to formulate the topic;systematically collect and organize the academic thoughts and clinical experience of Associate Professor *** in the diagnosis and treatment of pediatric pulmonary diseases in the form of interviews and clinical follow-up.2.Organize the case of Associate Professor Huang of diagnosing and treating with common pediatric pulmonary diseases;summarize the academic thoughts and clinical experience of the instructors in terms of type of treatment,identification points,prescription medication,and prognostic care,and list the corresponding medical cases after the type of treatment.[Results]:Part Ⅰ An overview of academic thought:1.Overall physiological and pathological characteristics(1)Physiological characteristics:(1)vigorous development;(2)organs are delicate,the tissues are not mature.(2)Pathological features:(1)easy onset,rapid metastasis;(2)clear spirit of organs and easy to recover;(3)more common in Yangming disease.2.Pediatric physiology and pathology of lungs(1)Lungs are directly connected to the outside through the airway in the anatomical structure.Physiologically,the lung is often in insufficiency,which is embodied in the form of delicate shape and lack of lung qi.(2)Its pathological features are as follows:(1)high incidence,easy to infect external evils;(2)the disease is more than easy and ambiguous;(3)easy to turn cold or heat,easily turn to vacuity or repletion;(4)easy to produce sputum;(5)the main symptoms are cough,asthma,phlegm;(6)easy to be accompanied with food accumulation.3.Experience of diagnosis,identificationTo grasp the pathography,the instructor value the inspection and auscultation.In inspection,he uses coughing sound in the clinic to judge the pattern type,shallow depth and so on.Cough characteristics include: cough site-pharynx,throat,lung;nature-tight,rapid,heavy turbid,tight,loose,etc;time characteristics-morning cough,night cough,sputum cough,etc;They are caused by different pathogenic factors like wind,cold,heat,dampness,dryness and fire.It invades different disease position such as hair and skin,meridians and viscera,and involves different levels of body fluid,qi,blood,etc.In inspection,he usually observe the color and the morpha of the throat to judge what are the pathogens.4.Methods of treatment(1)In principle,the treatment focuses on the situation,according to the disease to help the vital qi to fight against the evil,so that the disease won’t be left too long,For example,the treatment of cough focus on Freeing lung-qi,the treatment of diarrhea-treating the unstopped by unstopping.(2)Pay attention to the spleen and stomach,avoid using bitter cold,to hurt the spleen,and help the spleen work well by strengthening it.(3)On the prescription,the prescription is flexible and the medication is light.(4)Pay attention to the method of decoction.The younger ones use the method of steaming water to control the dosage while ensuring the effectiveness of the drug,which is also convenient for children to take.5.Experience of prognosis and trainingThe tutor strongly agrees with the nursing method of “To take care of children,three points of hunger and cold”,and further points out that “hungry” before the “cold”,so that the family members are instructed to look after the children,often with half the effort.The nursing methods of different diseases should also be taken seriously,such as coughing children,should be guided to actively sputum their sputum;Children with phlegm and dampness in the lung for too long,should be kept warm to prevent repeated infection and damage to the righteousness.Part Ⅱ Summary of experienceAccording to the main symptoms,the common pediatric lung diseases in the outpatient department are classified as follows:1.four types of nasal congestion,salivation syndrome-treatments: Nasal infection by wind-cold and depressed heat(sparing wind and sputum method),Rheumatism and stagnation in the nasal sputum syndrome(Cang Er Zi San and Ge Gen Qin Lian Tang),Skin syndrome caused by exogenous wind-cold(San Ao Tang),Deficiency of lung-defense with external wind-wet(Cang Er Zi San and Yu Ping Feng San),Deficiency of lung qi and spleen qi with external wind(Shen Su Yin).2.seven kinds of fever syndrome-treatments: cold-wind and heatstagnation in the lung(Ma Xing Shi Gan Tang),throat disability caused by wind and heat-stagnation on the upper burner(Yin Qiao San),cold-wind and heat-stagnation in the lung(San Ao Tang and Xing Ren Tang),wind-heat in the lung(Sang Ju Yin),wind-wet-heat in the Yangming channel(Gan Lu Xiao Du Dan),food-stagnation with the wind-cold evil outside(Huo Xiang Zhengqi San),accumulated stagnation with wind-heat outside(Bo Bin San);3.nine types of coughing syndrome-treatments:pharyngeal disability infected by wind(Zhi Sou San),damp-heat in Construction aspect of Shangjiao(Yin qiao San),damp-heat in qi aspect of Shangjiao(Shang Jiao Xuan Bi Tang),Wind-cold in the lung(Xing Su San),Wet-heat in the lung(Xing Ren Tang),Wind-heat in the lung(Sang Ju Yin),Phlegm in the lung with external wind-cold(Xing Su San),phlegm-heat in the lung(Qian Jin Wei Jing Tang),Deficiency of spleen qi with phlegm-dampness in the lung syndrome(Liu Jun Zi Tang);4.two kinds of asthma syndrome-treatments:Phlegm in the lung with exgenous wind-cold(Xing Su San),Deficiency of spleen qi with phlegmdampness in the lung syndrome(Liu Jun Zi Tang).[Conclusion]: Associate Professor Huang has formed a relatively objective diagnosis and treatment system for pediatric pulmonary diseases,and has adopted a rich theoretical foundation and treatment methods.By summarizing the clinical cases of Associate Professor Huang in the treatment of pediatric pulmonary diseases,it can be seen that the instructor adopted a systematic process of treating the disease,and the complete treatment method. |
