Abstract/Summary:
Objective :To evaluate the surgical and postoperative outcome of robot assisted surgery compared with conventional thoracotomy and video-assisted thoracic surgery in the early stage of NSCLC(non-small cell lung cancer)and to provide a reference for the surgical treatment.Methods: 118 patients with early-stage NSCLC hospitalized in First affiliated hospital of Xi’an JiaoTong university from Jan 2016 to Oct 2017 were enrolled in this paper,.According to the principle of voluntarily choose,52 patients were divided into conventional thoracotomy group,43 patients were divided into video-assisted thoracic surgery group and 23 patients were divided into robot assisted surgery group.The general situation of patients such as age,gender,weight and tumor size,TNM stage were respectively recorded before surgery,then every patients received operation according to the corresponding groups,all the surgery were undergoing smoothly.Then each patient’s surgery and postoperative related indicators such as operation time,intraoperative blood loss,number of lymph nodes,number of lymph node stations,extubation time,total drainage,postoperative hospital stay,total cost of surgery were recorded,and finally statistical analysis was conducted by SPSS 19.0 to compare the differences between each group.1.Results:(1)There was no significant difference(P> 0.05)between the basic data of patients in the robot assisted surgery group and those in the other two groups,and both of them were comparable.(2)Compared with conventional thoracotomy group,the volume of bleeding,postoperative extubation times,total drainage and postoperative hospital stay days in robotic assisted surgery group were significantly improved(P <0.05).(3)Compared with video-assisted thoracic surgery group,the volume of bleeding and elimination of lymph nodes were significantly improved(P <0.05).(4)Compared with the other groups,the cost of surgery in the robot-assisted surgery group was significantly increased(P <0.05).Conclusion: Robot-assisted surgery is superior to conventional thoracotomy in the control of intraoperative blood loss,postoperative extubation time,total postoperative drainage,andpostoperative hospital stay.While it is also superior to thoracoscopic surgery in the control of intraoperative blood loss,the number of lymph nodes removed,and the number of lymph nodes stations removed.And although robot-assisted surgery is at a relatively disadvantage in controlling the cost of surgery,the degree of pain and satisfaction of the surgery are better.Therefore,the problem of surgical cost is eliminated.Robot-assisted surgery has its unique advantages in the treatment of early non-small cell lung cancer,and it is worthy of clinical application.
