Multidisciplinary Team Led By Specialist Nurse In Breast Cancer Patients Research On The Application Of PICC Management

Objective: To explore the application effect of multidisciplinary team(MDT)led by specialist nurses in the management of peripherally inserted central venous catheter(PICC)on breast cancer patients,so as to provide reference for standardized management of clinical PICC nursing.Methods: The application effect of multidisciplinary team cooperation was compared by historical comparison.Fifty breast cancer patients who received PICC catheterization and routine nursing from November 2015 to July 2016 in a hospital were selected as the routine treatment group,and 50 breast cancer patients who received PICC catheterization and multidisciplinary team management mode from August 2016 to April 2017 were served as the MDT cooperation group.The success rate of puncture,indwelling time of catheter,incidence of complications,unplanned extubation rate,patient satisfaction and nurse satisfaction were compared between the two groups before and after the establishment of the team.Results: There were no significant differences in breast cancer type,catheter length and tumor stage between the routine treatment group and the MDT cooperation group(P>0.05).Comparing with routine treatment group,the success rate of PICC puncture in the MDT cooperation group was higher(P<0.05),the average indwelling time of the catheter in the MDT cooperation group was longer(P<0.01),the unplanned extubation rate of the catheter in the MDT cooperation group was lower(P<0.05).In the MDT cooperation group,except the incidence of medical adhesive related skin injury and thrombosis in catheter was lower than that in the routine treatment group(P<0.05),the other complications had no statistical significance(P>0.05),and the total complications had statistical significance(P<0.01).After extubation,the satisfaction of patients in the MDT cooperation group was higher than that in the routine treatment group(P<0.01),meanwhile the overall satisfaction of nurses in the MDT cooperation group was higher than that in the routine treatment group too(P<0.01).Conclusion: The application of multi-disciplinary team management mode led by Specialized Nurse in PICC management of breast cancer patients can improve the success rate of catheter puncture,satisfaction both patient and nurse,decrease the incidence of catheter-related complications and unplanned extubation rate,and prolong the catheter retention time.It provides practical experience for improving the quality of PICC nursing management and promoting the professional development of venous therapy,therefore has definite clinical guiding value.
