Objective: To study the correlation between serum heat shock proteins 90?(HSP90?)and alpha fetoprotein(AFP)and the clinical characteristics of patients with Hepatocellular carcinoma,HCC).To explore the value of HSP90? and AFP detection alone and in combination in the early diagnosis of HCC.Methods: From January 2016 to November 2018,164 patients(HCC group)with Hepatocellular carcinoma(HCC)were selected as the experimental group,and 120 patients(benign group)with benign liver diseases were selected as the control group.Serum HSP90? and AFP levels in the two groups were detected,and the differences in serum HSP90? and AFP levels in the two groups were compared by statistical methods to analyze the correlation between serum HSP90? and clinical characteristics of liver cancer patients.By analyzing Receiver operating characteristic curve(ROC)and its sensitivity,specificity,positive predictive value,negative predictive value and total coincidence rate in the diagnosis of liver cancer,the differences in Area Under curve(AUC)of ROC were compared to explore the diagnostic value of HSP90? in liver cancer.Results: 1.Serum HSP90? and AFP levels in the HCC group were higher than those in the benign group,with statistically significant differences(U=3739.0 and 2350.5,all P<0.001).2.In the HCC group,there was no significant difference in serum HSP90? and AFP levels between the patients and general clinical characteristics(age,gender,liver function grade,and whether HBeAg was positive or not)(P>0.05).3.In the HCC group,serum HSP90? and AFP levels were compared with the pathological characteristics(greatest tumor diameter,presence or absence of portal vein tumor thrombus,presence or absence of satellite lesion).Compared with patients with tumor diameter greater than or equal to 5cm or less,serum HSP90? level showed statistically significant difference(P<0.05),while AFP showed no statistically significant difference(P>0.05).Serum HSP90? and AFP levels were statistically significant in patients with and without portal vein tumor thrombus(all P<0.05).Patients with and without satellite lesions were compared,serum HSP90? level showed statistically significant difference(P<0.05),while AFP showed no statistically significant difference(P>0.05).4.In the HCC group,there was no significant difference in serum HSP90? and AFP levels when the number of tumors was one,two to three,or greater than or equal to four(all P<0.05).5.In the HCC group,serum HSP90? and AFP levels were significantly different between stage?,? and ? patients(all P<0.05).The results of the two groups were as follows: There was no significant difference in serum HSP90? and AFP levels between stage ? and stage ? patients(all P>0.05),Serum HSP90? and AFP levels were statistically significant in patients with stage ? and stage ?(all P<0.05),Serum HSP90? and AFP levels were statistically significant in patients with stage?and stage ?(all P<0.05).6.In the HCC group,differences in serum HSP90? and AFP levels were statistically significant when comparing patients with highly differentiated,moderately differentiated,and poorly differentiated tumors(all P<0.05).The results of the two groups were as follows: there was no significant difference in serum HSP90? and AFP levels between patients with highly differentiated and moderately differentiated tumors(all P>0.05),There was no significant difference in serum HSP90? and AFP levels between patients with moderately differentiated and poorly differentiated differentiated tumors(all P>0.05),Serum HSP90 and AFP levels were statistically significant for comparison between patients with high and low tumor differentiation(all P<0.05).7.AUC of HCC diagnosed by HSP90? and AFP was statistically significant(Z= 2.099,P<0.05),There was a statistically significant difference in AUC of HCC diagnosed by HSP90? and HSP90? combined with AFP(Z=5.97,P<0.05).There was no significant difference in AUC between AFP and HSP90? for HCC(Z=1.47,P > 0.05).The specificity and positive predictive value of AFP in the diagnosis of HCC were both higher than HSP90?,and the combined detection of AFP and HSP90? in the diagnosis of HCC(all P<0.05).The sensitivity,negative predictive value and total coincidence rate of the combined detection were higher than HSP90? and AFP respectively(all P<0.05).8.The difference of AUC between HSP90? and AFP in the diagnosis of SHCC was statistically significant(Z=2.85,P<0.05).Comparison of AUC between HSP90? and HSP90? combined with AFP in the diagnosis of SHCC showed statistically significant difference(Z=3.13,P<0.05).There was no significant difference in AUC between AFP and HSP90? combined with AFP in the diagnosis of SHCC(Z=0.88,P>0.05).The specificity,positive predictive value and total coincidence rate of AFP in the diagnosis of SHCC were both higher than HSP90?,and the combined detection of AFP and HSP90? in the diagnosis of SHCC(all P<0.05).The sensitivity of combined detection of SHCC was higher than HSP90? and AFP respectively(all P<0.05).The negative predictive value of combined detection was higher than that of HSP90? alone(P<0.05).9.Serum HSP90? level of HCC patients with positive AFP was higher than that of HCC patients with negative AFP,and the difference was statistically significant(P < 0.05).Conclusion: 1.HSP90? is related to the occurrence and development of HCC,and the expression in serum of HCC patients is significantly higher than that of benign patients,which can be used for the diagnosis of HCC.Serum HSP90? level was correlated with the clinical stage and malignant degree of liver cancer.2.The value of HSP90? in the combined diagnosis of liver cancer is higher than that of AFP alone.HSP90? is of certain value in the diagnosis of small liver cancer,and the accuracy of small liver cancer diagnosis with AFP is not significantly improved.It also has certain value for the diagnosis of AFP negative liver cancer.HSP90? can assist AFP in the diagnosis of hepatocellular carcinoma,which is expected to be a new serological marker of hepatocellular carcinoma.
