Backgroud:Colon cancer is the most common malignant tumor of the digestive system in the world,and its morbidity and mortality are increasing year by year.Most patients often have metastasis at the time of initial diagnosis,and the prognosis is bad,because of its initial diagnosis.EFEMP2,as a kind of extracellular matrix protein in tumor microenvironment,is associated with various tumorigenesis and development.This study aimed to investigate the expression and clinical significance of EFEMP2 in colon cancer tissues.Objective:To investigate the expression of EFEMP2 in colon cancer tissues,paracancer tissues and their matched liver metastatic tissues.To investigate the relationship between EFEMP2 expression in colon cancer tissues and the clinical,pathological characteristics of patients.And the clinical significance of this relationship.Methods:Retrospective study of surgical resection of colon cancer tissues,partial matched paracancerous and hepatic metastatic carcinoma tissues of colon cancer patients who underwent surgery in the General Surgery of Shaanxi Cancer Hospital from November 2006 to December 2018.And using immunohistochemistry technique SP method to detect the expression of EFEMP2 and Ki-67 in these tissues.H&E sections of colon cancer tissue and matched liver metastatic tissue specimens were taken from all colon patients.Evaluated the tumor stroma percentage(TSP)and tumor inflammatory infiltration Klintrup-M?kinen score(K-M score)of them.Collect clinical and pathological data of all colon patients,and record in detail their age,gender,whether other tumors were combined,whether there were intestinal obstruction before surgery,and their tumor markers(CA-50,CA-199,CEA,CA242,AFP),blood routine(WBC,N,NEUT,RBC,HB,PLT),liver function(TBIL,DBIL,IBIL,TCHO,TP,ALB),TNM staging,lymph node metastasis,tumor location,tumor histology type,mismatch repair protein(MLH1,MSH6,PMS2,MSH2),multidrug resistance associated protein(TS,Pgp,GSTπ,ERCC1,LRP).All colon patients were reviewed and registered by electronic medical record retrieval,and a telephone interview was conducted.The follow-up date was until March 18,2019.and the median follow-up time was 11.68 months.Using SPSS18.0 statistical software to analysis the data results,the correlation between the expression of EFEMP2 and TSP in tissues and their clinical and pathological parameters was determined by chi-square test.EFEMP2 expression in colon cancer tissues and its matched paracancerous and liver metastatic cancer tissues using a two-tailed t-test,TSP and K-M scores were compared between colon cancer tissues and their matched liver metastases using a two-tailed t-test.P<0.05 was considered statistically significant.Results:The expression of EFEMP2 in colon cancer tissues was significantly lower than that in adjacent tissues(t=-2.472,P﹤0.05),and the expression of EFEMP2 in liver metastasis tissues was significantly lower than that in primary colon cancer tissues(t=3.688,P﹤0.05);In colon cancer patients,the expression of EFEMP2 and TSP(χ~2=7.862,P<0.01),Ki-67(χ~2=4.397,P<0.05),LRP(χ~2=6.094,P<0.05)was negatively correlated.The TSP score(t=-3.000,P﹤0.05),K-M scores(t=-4.610,P﹤0.05)in colon cancer tissues was significantly lower than that in liver metastasis tissues.TSP-H was positively correlated with lymph node metastasis(χ~2=4.321,P<0.05)and pathological stage(χ~2=1.920,P<0.05).Conclusion:EFEMP2 may inhibit the development of colon cancer,and it is a cancer suppressor in colon cancer.Tumor stroma and inflammatory tumor microenvironment play an important role in the invasion and metastasis of colon cancer.
