| Objective: By observing the clinical case data of patients with advanced epithelial ovarian cancer who underwent neoadjuvant chemotherapy combined with intermittent cytoreductive surgery,to explore the predictive effect of serum CA125 on the satisfaction rate and prognosis of neoadjuvant chemotherapy,in order to provide data support for the development of treatment plans and prognosis in patients with advanced epithelial ovarian cancer.Methods: Retrospective analysis of clinical case data of 76 patients with advanced epithelial ovarian cancer who underwent cytoreductive surgery after 3cycles of neoadjuvant chemotherapy in Ningxia Medical University General Hospital from January 1,2013 to December 31,2017.The correlation between serum CA125 value at the initial diagnosis,serum CA125 decrease rate after first chemotherapy,preoperative serum CA125 value and CA125 half-life and tumor cytoreductive surgery satisfaction rate(postoperative residual tumor diameter<1cm)were analyzed.The tumor cell satisfaction rate,FIGO stage,histological grade,preoperative serum CA125 value,serum CA125 decline rate after first chemotherapy and CA125 half-life were included in the COX proportional hazards regression model to determine the independent prognostic factors of patients with advanced epithelial ovarian cancer.Results: The rate of serum CA125 decreased by ≥70% after neoadjuvantchemotherapy in the first course of treatment and the satisfaction rate of tumor reduction in <70% group was no statistical difference(P<0.05).There was no statistical difference in the median overall survival and progression-free survival(P<0.05).There was a statistically significant difference between the preoperative serum CA125≤100U/ml group and the >100U/ml group(P<0.05).There was no significant difference in the median overall survival and progression-free survival between the two groups(P>0.05).There were significant differences in the satisfaction rate,median survival time and progression-free survival of serum CA125 half-life ≤20 days group and >20 days group(P<0.05).COX proportional hazards model analysis showed that the tumor reduction satisfaction rate(HR=2.626,95% CI:22.186~37.814,P=0.044),serum CA125 half-life(HR=1.030,95% CI:1.010 ~ 1.051,P=0.004),they are the independent prognostic factors of advanced epithelial ovarian cancer.Conclusion: 1.Whether the preoperative serum CA125 value is ≤100U/ml,whether the CA125 half-life is ≤20 days,whether the serum CA125 decline rate after neoadjuvant chemotherapy is ≥70% in the first course of treatment can be used to preliminarily determine the tumor reduction of tumor cytoreductive surgery after neoadjuvant chemotherapy effect.2.The satisfaction of tumor reduction and CA125 half-life are independent factors influencing the prognosis of patients with advanced epithelial ovarian cancer. |
