Objective: To investigate the relationship between neutrophil lymphocyte ratio and heart failure and its severity.METHODS: A total of 152 patients with heart failure who were hospitalized in the heart center of the First Hospital of Lanzhou University from December 2015 to January 1919 were enrolled as a heart failure group.According to the New York Heart Function Classification,they were divided into three groups: NYHA class II,31 cases.There were 54 cases of NYHA class III and 67 cases of NYHA class IV.50 healthy subjects with average age and average age of heart failure group were collected as healthy controls.The basic data of each group were collected: blood routine(absolute white blood cells,absolute neutrophils,absolute lymphocytes),NT-proBNP,echocardiographic related indicators(LVEF),etc.,and the ratio of neutrophil lymphocytes was calculated.(NLR).First,compare the baseline data and NLR differences between the heart failure group and the healthy control group,and analyze the predictive value of NLR for the diagnosis of heart failure by plotting the receiver operating characteristic(ROC)curve.Secondly,compare the differences between different cardiac function classifications.Differences between baseline data and NLR,independent risk factors for assessing the severity of heart failure by ordered multivariate logistic regression analysis;and finally,because NYHA class IV patients were considered to be the end stage of heart failure,mortality and rehospitalization rates for such patients High,the predicted value of NLR,NT-proBNP for the diagnosis of severe heart failure(NYHA class IV)was analyzed by plotting the receiver operating characteristic(ROC)curve.RESULTS:(1)Compared with the healthy control group,the NLR level in the heart failure group was significantly increased,and the difference between the two groups was statistically significant(P<0.05).The ROC curve analysis showed that NLR has reference value for the diagnosis of heart failure.(2)The chi-square test andKruskal-Wallis nonparametric test analysis showed that the NLR and the levels were statistically significant(P<0.05)between NYHA class II,NYHA class III and NYHA class IV(P<0.05).Decreased cardiac function and elevated NLR levels;(3)Logistic regression analysis showed that NLR was an independent predictor of severity of heart failure;(4)ROC curve analysis showed that the area under the curve of NLR predicting severe heart failure was 0.73(P < 0.001),the 95% confidence interval was0.625-0.801,and the optimal cut-off value was 1.38.This value corresponds to a diagnostic sensitivity of 62.7% and a specificity of 74.1%.CONCIUSION:The results of this study showed that the level of NLR in patients with heart failure was higher than that in healthy subjects,and it was positively correlated with NT-proBNP and NYHA cardiac function.NLR may be a reference factor for predicting HF and its severity.
