Objective:To study the protective effect of esmolol,a short-acting beta-1 receptor blocker,on brain tissue in sepsis rats.Method:Forty-eight SD male rats(250g-300g)were divided into three groups according to random number table: sham group + saline group(Sham group),sepsis group + saline group(CLP group)and sepsis + esmolol group(ES group).Four rats in each group were randomly selected and labeled as group A,in order to observe the neurobehavioral scores of the remaining 12 hours and 24 hours after modeling.Twelve rats in group B were randomly divided into three subgroups: 6-hour group,12-hour group and 24-hour group.In the CLP group and the ES group,the sepsis model was established by cecal ligation and puncture.The Sham group was treated with the same method but without cecal ligation and puncture.Right femoral vein catheterization was performed in each group after successful modeling.Sham group and CLP group were pumped with 0.9% normal saline 1ml/h for 6h;ES group was pumped with esmolol injection 1ml/h(15mg/kg/h)for 6h.Rats in group B were executed at 6,12 and 24 hours after successful modeling and serum samples were collected.Serum levels of interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-alpha)and ubiquitin carboxy terminal hydrolase L1(UCH-L1)were detected by ELISA.The pathological changes of brain tissue were observed through the optical microscope.Result: 1.1 Comparison of different groups of rats at the same time point: Compared with Sham group,the levels of TNF-alpha and IL-6 in serum of ES group and CLP group increased significantly at 6,12 and 24 hours after operation(P < 0.05);compared with CLP group,the levels of TNF-alpha and IL-6 in serum of ES group decreased significantly at 6,12 and 24 hours after operation(P < 0.05)1.2 Compared with rats in the same group at different time points,the level of serum TNF-a in Sham group had no statistical significance.The level of TNF-a in CLP group increased at 24 h,6 h,24 h and 12 h(P < 0.05).The level of TNF-a in ES group increased at 24 h and 6 h(P < 0.05).There was no statistical significance between the remaining time points.Serum IL-6 level: The levels of IL-6 in Sham group increased significantly at 24 h and 6 h(P < 0.05),but there was no statistical significance between the remaining time points.The levels of IL-6 in CLP group increased at 24 h,12 h,12 h and 6 h(P < 0.05).The levels of IL-6 in ES group increased at 24 h and 12 h,12 h and 6 h(P < 0.05)2.1 Comparison of different groups of rats at the same time point: Compared with Sham group,UCH-L1 level in ES group and CLP group increased significantly at 6 h,12 h and 24 h after operation(P < 0.05);compared with CLP group,UCH-L1 level in ES group decreased significantly at 6 h,12 h and 24 h after operation(P < 0.05)2.2 Compared with rats in the same group at different time points:there was no significant difference between Sham and CLP groups at different time points,and UCH-L1 levels in ES group increased at 12 and 6 hours(P < 0.05).The level of UCH-L1 decreased significantly at 24 h and 12 h(P < 0.05).3.Compared with Sham group rats at 12 and 24 hours after operation,neurobehavioral scores of ES group and CLP group rats decreased significantly at corresponding time points after operation,the difference was statistically significant(p < 0.05);compared with CLP group rats at 12 and 24 hours after operation,neurobehavioral scores of ES group rats increased significantly at corresponding time points after operation,the difference was statistically significant(p < 0.05).4.Pathological results of brain tissue: the pathological morphology of brain tissue in Sham group was as follows: mild cerebral edema,glial cell proliferation was not obvious and a few nuclei were hyperchromatic;The pathological morphology of brain tissue in CLP group was as follows:edema of brain tissue,a large number of glial cells proliferated,more hyperchromatic nuclei could be seen,perivascular edema widened.Compared with CLP group,ES group showed slight changes..Conclusion: Esmolol inhibits the release of inflammatory factors in the body,reduces the level of inflammatory factors in serum,improves the inflammatory response of brain tissue,reduces the degree of brain tissue damage in sepsis,and protects the brain tissue of septic rats.
