Objective:This study investigated the value of intravoxel incoherent motion diffusion weighted imaging?IVIM-DWI?in the differential diagnosis of benign and malignant cervical lymph node lesions.Methods:The patients with malignant lymph nodes and benign lymph nodes confirmed by operation or pathology in the First Affiliated Hospital of China Medical University were collected in 14 cases and 12 cases respectively.All patients underwent conventional MRI plain scan and IVIM with GE 3.0T magnetic resonance spectroscopy.-DWI sequence scan;the b value were:0,10,50,100,150,200,400,600,800,1000s/mm2,2doctors with more than 5 years of experience in imaging diagnosis The lesions showed a clear maximum level of delineation of the appropriate region of interest?ROI?,using the GE Functool MADC post-processing software to obtain standard ADC values,D values,D*values,and f values for the benign and malignant lesions,respectively;using independent sample t-test The statistical significance of the mean values of the parameters between the two groups was compared.?=0.05 was used as the test level,and the difference was statistically significant at P<0.05.The receiver operating characteristic curve?ROC?was used to compare the diagnostic efficacy of each parameter for lymph node malignant lesions.Results:1.The standard ADC values of malignant lymph node group and benign group were:?0.77±0.09?×10-3s/mm2,?1.05±0.28?×10-3s/mm2,and the standard ADC value of malignant group was lower than that of benign group.Statistically significant between them?t=3.53,P<0.05?;the D values between the malignant lymph node group and the benign lymph node group were?0.51±0.13?×10-3s/mm2,?0.76±0.12?×10-3s/mm2,respectively.Statistically significant differences between them?t=5.55,P<0.05?.The f-values of the malignant lymph node group and the benign lymph node group were?0.46±0.08?and?0.28±0.07?,respectively.The mean f-value of the malignant group was higher than that of the benign group.Statistically significant differences between them?t=6.9,P<0.05?.2.The D*values of malignant lymph node group and benign lymph node group were?7.32±2.98?×10-3s/mm2?11.46±4.32?×10-3s/mm2,respectively.No significant difference between them?t=3.09,P=0.05?.3.The standard ADC value between the two groups is 0.931,the sensitivity is78.6%,the specificity is 93.8%,the diagnostic threshold is 0.89×10-3s/mm2,and the area under the RO value curve is 0.920.The sensitivity is 78.6%,the specificity is 93.8%,the diagnostic threshold is 0.67×10-3s/mm2;the area under the RO value curve is 0.982,and the sensitivity and specificity of the f value are 100%and 93.7%,respectively.The diagnostic threshold is 0.38.Conclusion:The standard ADC and D values of the malignant lymph node group are lower than those of the benign lymph node group.The perfusion score f of the malignant lymph node group is higher than that of the benign lymph node group.IVIM-DWI can quantitatively evaluate the diffusion and perfusion characteristics of the two lesions,so IVIM-DWI has a high differential diagnosis value for cervical lymph node lesions;and the f-value has higher diagnostic efficiency,better sensitivity and specificity,and is of great value in the differential diagnosis of malignant lymph node lesions.
