Objective:To analyze the prognosis of Kasai surgery and related factors of postoperative cholangitis.Methods:The clinical data of 80 patients which undergoing Kasai portoenterostomy at shengjing hospital between November 2012 to March 2018 were retrospectively analyzed.Exploring the effects of different factors which inclued age,preoperative direct bilirubin,GGT,degree of liver fibrosis and degree of inflammatory infiltration,jaundice regression,recovery of liver function for Kasai prognosis and cholangitis.Results:Univariate analysis found that the age of surgery,sex,degree of liver fibrosis,degree of inflammatory infiltration,jaundice regression,liver function recovery were significantly associated with autologous liver survival.Preoperative GGT was associated with cholangitis(P<0.05);preoperative GGT,jaundice regression,liver function recovery have relation with cholangitis occurred sooner or later,and poor liver function recovery have 2.67 times will occur in early cholangitis.Operative age difference(66.74±17.87 vs 58.79±11.45d),jaundice subsidence,liver function recovery and frequency of cholangitis have relation with cholangitis,and poor liver function recovery have 2.54 times will occur in frequent cholangitisConclusions:The age of the operation,the degree of inflammatory infiltration,the regression of jaundice and the recovery of liver function were the main factors affecting the survival of autologous liver.the recovery of liver function have the greatest influence to the survival of autologous liver.GGT is a risk factor for cholangitis.High level of GGT,jaundice is not regressed,poor liver function recovery is a risk factor for early cholangitis.The recovery of liver function affects the time of cholangitis.Operative age,jaundice,and poor recovery of liver function are risk factors for frequent cholangitis.The recovery of liver function is the most factors to affects the frequency of cholangitis.
