Predictive Significance Of Gleason Score On Prostate Cancer Outcome

Objective: Prostate cancer is a high incidence of male malignant tumor,patients diagnosed easy progress after castration treatment for castration resistant prostate cancer,now still lack of clinical can predict the reliability index of the progress of castration resistant prostate cancer.This study is to obtain information from the clinical cases,to explore the relationship between Gleason score and Preliminary prostate-specific antigen,prognosis,bone metastasis and CRPC progression in prostate cancer patients?Methods: A total of 88 cases of pathological diagnosis of prostate cancer were collected from shengjing Hospital of China Medical University from January 1,2012 to December 31,2013.The follow-up time was as of June 3,2017.The patients were divided into two groups according to the Gleason score(the middle and low risk group was less than 7 and the high risk group > 7).Spearman level correlation analysis,chi-square test,Kaplan Meier survival analysis and log-rank test statistics methods were adopted,discuss the relationship between two groups of Gleason scores on survival time,CRPC progressionfree time and bone metastasis in patients with prostate cancerResults: From 88 cases of patient data,Gleason score was positively correlated with TPSA(r= 0.595,P< 0.01).56 people in the middle and low risk group had 14 bone metastases,32 people in the high risk group had 18 bone metastases.The probability of bone transfer between the two groups was significantly different(?2=8.594,P =0.003).The number of total samples / death events in the two groups was 56/12 and 32/18 respectively,and the OS(95% CI)in the high risk group was 49 months(38.9 months ~59 months).Middle and low risk group OS still need follow-up.The difference of survival time between the two groups was statistically significant(?2 =12.25,P =0.01);In terms of treatment methods,none of the patients who received radical surgery or radical surgery combined with castration or androgen deprivation therapy have died of prostate cancer.Sixty-three men who had been treated with castration combined with androgen deprivation therapy and 17 died of prostate cancer.Among the patients with endocrine therapy,39 people in the middle and low risk group and 24 people in the high risk group,the number of CRPC progression was 18 and 17 respectively.The median time of no progression in the middle and low risk group was 60 months(46.474~73.52).and in high risk group was 36 months(16.97~55.02).The time difference of CRPC progression free time in the two groups was statistically significant(?2=7.223,P =0.007).Gleason score was positively correlated the score,the higher the probabi Conclusion: with the initial TPSA.The higher lity of bone metastasis,the decrease in survival time and the shorter duration of CRPC.
