Objective: To study the correlation between vaginal cleanliness and recurrence and Human papillomavirus infection after cervical conization for high-grade cervical intraepithelial neoplasia.Methods: The follow-up data of 128 patients with high-grade cervical intraepithelial neoplasia such as CINII-CINIII were reviewed at 24 months after cervical conization,including 16 patients with recurrence(study group),112 patients without recurrence(control group),69 patients with I-II degree of vaginal cleanliness at 6 months after surgery,and 59 patients with III-IV degree.The study is to analyze the relationship between the test results of vaginal secretion cleanliness at 6 months postoperatively and the recurrence of HPV infection and high-grade cervical intraepithelial neoplasia after cervical conization.Results: Among the 128 patients,16 had recurrence rate of 12.500%.1.The recurrence rates of vaginal cleanliness I-II and III-IV at 6 months after surgery were 2.899%(2/69)and 23.729%(14/59),respectively,that is,the postoperative recurrence rates of patients with III-IV degree of vaginal cleanliness were significantly higher than those with I-II degree of vaginal cleanliness.The III-IV degree of vaginal cleanliness at 6 months after surgery was correlated with postoperative recurrence of high-grade cervical intraepithelial neoplasia(P<0.05).Postoperative HPV infection and positive resection margin were also correlated with the recurrence of high-grade cervical intraepithelial neoplasia after cervical conization(P<0.05).2.The rates of HPV positivity were 7.246%(5/69)and 30.508%(18/59)for patients with grade I-II vaginal cleanliness and grade III-IV vaginal cleanliness 6 months after surgery,respectively.That is to say,postoperative HPV positive rates for patients with grade III-IV vaginal cleanliness were significantly higher than those with grade I-II vaginal cleanliness.Vaginal cleanliness level III-IV 6 months after surgery was correlated with HPV infection after high-grade cervical intraepithelial neoplasia(P<0.05).3.Multivariate logistic regression analysis showed that abnormal vaginal cleanliness 6 months after surgery was not an independent risk factor for recurrence of high-grade cervical intraepithelial neoplasia(P > 0.05).HPV infection and positive resection margin were independent risk factors for recurrence of high-grade cervical intraepithelial neoplasia after cervical conization(P < 0.05).Conclusion: Abnormal vaginal cleanliness at 6 months after surgery was associated with recurrence and HPV infection in patients with high-grade cervical intraepithelial neoplasia after cervical conization.Abnormal vaginal cleanliness 6 months after surgery is not an independent risk factor for recurrence of high-grade cervical intraepithelial neoplasia.Vaginal cleanliness may lead to postoperative recurrence of high-grade cervical intraepithelial neoplasia by exacerbating postoperative HPV infection,but the two are not directly related.
