Objective:Mycobacterium tuberculosis(MTB)is a pathogenic pathogen of tuberculosis,and its clinical drug resistance is becoming more and more common.The traditional drug resistance test is mainly based on in vitro drug sensitivity test.However,this method has a long cycle,and the diagnostic information is lagging.Provide a basis for diagnosis and treatment.In this study,the gene-chip technology was used to rapidly detect the related genes of rifampicin and isoniazid-resistant genes of Mycobacterium tuberculosis,and provide timely and clear diagnosis and treatment basis for the clinic.Methods:This study collected 934 specimens of patients diagnosed with tuberculosis in the outpatient department and hospitalization of the Fifth People's Hospital of Wuxi,Jiangsu Province from October 2017 to October 2018,and 90 samples of the strains of mycobacterium tuberculosis in vitro drug susceptibility laboratory provided by the Chinese center for disease control and prevention.Gene chip technology was used to detect gene mutations of rifampicin rpoB gene 511,513,516,526,531,533 and isoniazid katG gene 315 and inhA gene-15 in Mycobacterium tuberculosis.On the basis of drug sensitive test in vitro.Using chi square test and comparative analysis Results:Among the 934 samples,109 were positive for drug resistance mutations,of which 23 were positive for rifampicin alone,51 were positive for isoniazid resistance,and 35 were positive for double drug resistance.The rpoB locus mutation of rifampicin in Mycobacterium tuberculosis in Wuxi was mainly 531(35 cases,32.1%),followed by 526(9 cases,8.3%),resistance to isoniazid katG315 mutation There were 62cases(56.9%)positive and 24 cases(22.0%)with positive mutation of inhA gene-15.In these clinical samples,the results of gene chip detection and in vitro drug susceptibility test showed that the positive coincidence rate of rifampicin resistance and isoniazid resistance was 90.32% and80.41%,respectively.With the interventricular quality assessment samples provided by the Chinese center for disease control and prevention as the gold standard,the sensitivity,specificity,82.35% and specificity of rifampin resistance detection by gene microarray were 93.75%,95.24%,82.35% and 92.31%,respectively.Conclusion:This study found that genotypic prediction of the sensitivity of Mycobacterium tuberculosis to rifampicin and isoniazid is highlycorrelated with phenotypic resistance of these drugs.The gene chip technology can efficiently detect the mutation of rifampicin and isoniazid-resistant gene of Mycobacterium tuberculosis,and provide clinical basis for diagnosis and treatment.
