Objective: This experiment explored the expression of podocalyxin and nephrin in the urine of patients with different pathological types of primary nephrotic syndrome,and analyzed the relationship between them and the clinical indexes(urinary protein,albumin,blood lipid,creatinine,etc.)so as to provide a basis for the rapid diagnosis of primary nephrotic syndrome and more accurate guidance or prognosis.Methods:(1)The urine of 80 patients with primary nephrotic syndrome diagnosed clinically and pathologically was collected in the Department of Nephrology at Affiliated Hospital of Youjiang Medical University for Nationalities and People's Hospital of Baise from August 2016 to August 2017.Based on pathological classification all patients were divided into four group:minimal change nephrosis ? mesangial proliferative glomerulonephritis?membranous nephropathy and focal segmental glomerulosclerosis.The number of patients in each group was 22,11,36 and 11 respectively.The urine of 20 healthy persons was also collected as a control group.(2)The contents of podocalyxin and nephrin in urine of patients with different pathological types of nephrotic syndrome and normal control group were detected by enzyme linked immunosorbent assay.(3)The levels of 24-hour urinary protein,albumin,blood lipids,and creatinine form primary nephrotic syndrome patients and control subjects were collected to analyse their correlations with podocalyxin and nephrin.Results:(1)The expression of podocalyxin in urine of different groups was positively correlated with 24-hour urinary protein(r=0.60,P<0.05),negatively correlated with albumin(r=-0.47,P<0.05),and positively correlated with cholesterol(r=0.44,P<0.05),positivelycorrelated with triglycerides(r=0.48,P<0.05)and positively correlated with creatinine(r=0.24,P<0.05).(3)The expression of nephrin in urine of different groups was positively correlated with 24-hour urine protein(r=0.56,P<0.05),negatively correlated with albumin(r=-0.58,P<0.05),and positively correlated with cholesterol(r=0.37,P<0.05),positively correlated with triglycerides(r=0.32,P<0.05);positively correlated with creatinine(r=0.30,P<0.05).Conclusion:(1)Podocalyxin and nephrin are all expressed in human urine.The expression of podocalyxin?nephrin in the primary nephrotic syndrome group are higher than the normal group.Podocalyxin and nephrin expression are different in PNS urine of different pathological types.Podocalyxin and nephrin expression is highest in FSGS,nephrin's expression is an increasing trend in the severity of expression with pathological type,suggesting that the level of these two proteins can distinguish the different pathological types of renal syndesmosis.(2)Podocalyxin is positively related to 24 hours urine protein,blood lipid and creatinine,but the relation of podocalyxin and albumin is a negative correlation.This experiment prove that podocalyxin may contribute to the diagnosis of primary nephrotic syndrome.(3)Nephrin is positively related to 24 hours urine protein,blood lipid and creatinine,but the relation of nephrin and albumin is a negative correlation.This experiment prove that nephrin may contribute to the diagnosis of primary nephrotic syndrome.
