Objective:Retrospective analysis and Comparison of clinical efficacy of titanium elastic nailing(TEN)and plate internal fixation in the treatment of diaphyseal both-bone forearm fractures.Method: We totally collected all cases(38 Patients)of diaphyseal both-bone forearm fractures treated with TEN and plate internal fixation between 2012 and 2017 in our hospital.Grouping cases through different treatments,and there were 15 cases in group TEN and 23 cases in plate group.The operative time,blood loss,time to union,Radial bow magnitude,Radial bow location,forearm rotation function were compared between the two groups.All continuous variables were analyzed using the t test,including two sample t test and single sample t test,two-sided P values<0.05 were considered statistically significant difference.All categorical variables were analyzed using Fisher exact test,two-sided P values<0.01 were considered statistically significant difference.Results:The mean operating time was(76.9±36.8)min in the TEN group,the mean operating time was(133.8±43.3)min in the plate group;The mean blood loss was(29.0±28.4)ml in the TEN group,the mean blood loss was(52.2±37.2)ml in the plate group;The mean time to union was(39.1±16.4)w in the TEN group,the mean time to union was(53.9±20.4)w in the plate group;The mean magnitude of the maximum radial bow was(5.57±0.65)mm in the TEN group,the mean magnitude of the maximum radial bow was(6.52±1.21)mm in the plate group;The mean location of the maximum radial bow was(67.20±5.58)mm in the TEN group,The mean location of the maximum radial bow was(66.48±6.24)mm in the plate group;Theexcellent and good rate of rotating function is 100% both in the TEN group and the plate group.The two groups were statistically significant difference in the operating time,blood loss,time to union and radial bow magnitude(P<0.05).There was no statistically significant difference between the two groups in the location of the radial bow(P<0.05)and the function of forearm rotation after operation(P<0.01).Statistically significant difference was found for the mean magnitude of the maximum radial bow between the two groups and the normal value,but the plate group is close to the normal value(P<0.0 5).Statistically significant difference was found for the mean location of the maximum radial bow between the two groups and the normal value,but the plate group is close to the normal value(P<0.05).Conclusion:The intramedullary nailing group was both superior to the plate internal fixation group in the operation time,the amount of blood loss during the operation and the healing of the fracture after the operation.But,there was no statistically significant difference between the elastic intramedullary nailing group and the plate fixation group in the forearm rotation after the operation.To summarise,both the elastic intramedullary nailing and the internal fixation of the steel plate are reliable,safe and effective treatment for children,adolescents and adults.Furthermore,the elastic intramedullary nailing is also a good alternative to plate internal fixation,it is worthy of clinical application and popularization.
