Objective To investigate intravenous lidocaine lidocaine in patients undergoing thoracoscopic lobectomy,and to observe the intraoperative anesthetic dose,postoperative pain score,intraoperative and postoperative complications in the two groups,and to analyze the effect of intravenous lidocaine on enhanced recovery after surgery?ERAS?.Methods Eighty patients underwent elective thoracoscopic lobectomy were randomly divided into two groups?n=40?.In the lidocaine group(patients continuously received intravenous pumping of lidocaine 2 mg·kg-1·h-1after a loading doses 1.5 mg/kg until the end of the operation.)and control group?administered with the same amount of saline by the same way during surgery?.The same drugs and methods are used for induction of anesthesia and maintenance of anesthesia.The general conditions of the two groups of patients were recorded.Mean arterial pressure?MAP?,heart rate?HR?,and bispectral index?BIS?values were obtained.The doses of propofol and remifentanil in the operation,the incidence of arrhythmia,the incidence of postoperative nausea and vomiting,the length of postoperative hospital stay were recorded and the visual analog scale?VAS?were recorded.The patients in the two groups were compared at immediately after surgery?T1?,30 minutes after entering the PACU?T2?,2 hours after the operation?T3?,4 hours after the operation?T4?,8 hours after the operation?T5?,12 hours after the operation?T6?,and 24 hours after the operation.?T7?,48 hours after surgery?T8?pain.Results There were no significant differences in gender,age,BMI,ASA grade,operative time,anesthesia time between the two groups?P>0.05?.There was no significant difference in mean arterial pressure and heart rate between the two groups at each time point?P>0.05?.The BIS value of the lidocaine group was lower than that of the control group before extubation?P<0.05?.Comparison of intraoperative anesthetic doses between the two groups,the doses of remifentanil and propofol in the lidocaine group was lower than that of control group?P<0.05?.There was no significant difference in the incidence of arrhythmia between the two groups?P>0.05?.There was no significant difference in postoperative nausea and postoperative vomiting between the two groups?P>0.05?.Postoperative hospital stay was compared with postoperative hospitalization in the lidocaine group.The time was less than the control group?P<0.05?,and the difference was statistically significant.The postoperative analgesic effect of the two groups was compared.The VAS scores of lidocaine group were lower than those in the control group at immediately extubated,30 minutes after PACU,2hours after surgery,4 hours after surgery,8 h after surgery,12 h after surgery,and 24 h after surgery?P<0.05?.There was no significant difference in VAS scores between the two groups at 48 h after surgery?P>0.05?.ConclusionIntravenous lidocaine in patients undergoing thoracoscopic lobectomy can reduce the doses of remifentanil and propofol during surgery,reduce postoperative pain,shorten hospital stay and accelerate recovery and discharge.
