Objective To evaluate myocardial mechanics of coronary artery disease(CAD)patients without regional wall motion abnormalities(RWMA)after coronary artery bypass grafting(CABG)by three-dimensional speckle tracking imaging(3D-STI).Methods A total of 36 patients with CHD confirmed by coronary angiography who successfully underwent CABG surgery without segmental wall motion ab normalities from 2D-echocardiography were enrolled in the research.36 healthy subjects with matched age and sex were selected randomly as control group.Routine echocardiography and 3D-STI were performed 1 week before and 1we ek,3 weeks,and 1 month after the CABG.Then the conventional ultrasound parameters like left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),stroke volume(SV)were obtained respectively and the parameters of 3D-STI like global longitudinal strain(GLS),global radial s train(GRS),global circumferential strain(GCS),global area strain(GAS)and 3D-strain were obtained respectively too.Results Compared to the control group,LVEF,SV and LVEDD has no signi ficant difference of CABG group during 1 month of follow-up(P> 0.05).The absolute values of GRS,GLS,3D-strain and GAS in CABG group were signi ficantly decreased(P <0.05).The absolute value of GCS in CABG group was lower than that in control group,but the difference was not statistically signif icant(P > 0.05).Compared with CHD patients at 1 week preoperative,except GCS,all 3D-STI values decreased at 1 week postoperative but difference of a ll values was no significant(P> 0.05).Compared with CHD patients at 1 wee k preoperative,The GAS increased 3 weeks after operation(P> 0.05).The abso lute values of GRS,GLS and 3D-strain increased slightly,but the difference was not statistically significant(P > 0.05).All 3D-STI parameters were signific antly changed at 1 month preoperative except for GCS(P < 0.05),but there had significant difference compared with the control group(P <0.05).Conclusion 3D-STI can rapidly,intuitively and quantitatively analyze myocardial mechanics in patients with coronary heart disease.For patients with coronary artery disease without obvious RWMA,there is no significant difference in GCS of CABG patients after surgery Compared with that in 1 weak preoperative,The rest 3D-STI values of left ventricular were gradually improved.3D-STI can be applied to the quantitative analysis of myocardial mechanics before and after CABG surgery in patients without RWMA and the evaluation of the therapeutic effect.
