Objective:To investigate the role of TFF3 in evaluating the activity of inflammatory bowel disease(IBD)in children and its clinical diagnosis by detecting the levels of TFF3 in different disease classifications and stages,comparing the relationship between TFF3 and disease activity score,endoscopic mucosal score,and various laboratory indicators.Predict and judge the prognosis.Methods:Twenty-two/51 cases of inflammatory bowel disease in pediatric digestive department of Shengjing Hospital affiliated to China Medical University were selected as the study subjects.The visiting time was from March 2017 to September 2018.According to the degree of disease activity,the patients were divided into IBD active group and IBD clinical remission group.According to the type of disease,they were divided into Crohn's disease(CD)and ulcerative colitis(UC).According to the disease score,they were divided into four groups:CD active group,CD clinical remission group,UC active group and UC clinical remission group.Twenty children with health examination in pediatric department at the same period were selected as blank control group.Serum TFF3 was detected by ELISA,and clinical disease activity index(PUCAI,Pediatric ulcerative colitis activity index/PCDAI,Pediatric Crohn's disease activity index)was used to record laboratory indicators.Endoscopic score(Baron/SES-CD)was performed on patients who completed endoscopy.The difference of serum TFF3 in each group was counted.The correlation between serum TFF3 and endoscopy score and disease activity index was calculated.The diagnostic efficacy of TFF3 for disease activity and mucosal healing was evaluated.Results:1.Compared with the healthy control group,the serum TFF3 level in the IBD activity group was significantly increased,and the difference was statistically significant.There was no significant difference in serum TFF3 levels between the IBD group and the healthy control group.There was no significant difference in serum TFF3 levels between the CD and UC groups.2.According to the disease classification,compared with the CD clinical remission group,the serum TFF3 level in the CD activity group was significantly increased,and the difference was statistically significant.Compared with the UC clinical remission group,the serum TFF3 level in the UC activity group was significantly higher,and the difference was statistically significant.3.In patients with CD,serum TFF3 increased with the increase of endoscopic score of SES-CD,and serum TFF3 was positively correlated with SES-CD score.In patients with UC,serum TFF3 was not associated with endoscopic scores and was positively correlated with clinical disease score PUCAI.4.Serum TFF3 has a sensitivity of 100% for monitoring CD disease activity and a specificity of 76.2%.Conclusion: Serum TFF3 level can help to diagnose IBD and evaluate the activity of IBD in children.In CD,serum TFF3 level can reflect the degree of mucosal healing,which has guiding significance for the diagnosis and prognosis of IBD.
