With the rapid advancement of urbanization and the evolution of urban spatial structure,the contradiction between the demand for medical services and the supply of medical resources is getting worse due to the urban “inverted pyramid” style medical resource allocation pattern.In the context of big data,real-time traffic congestion data,hospital data,community data,and map service data are easily accessible.This provides more data support for fine-grained spatial accessibility analysis based on dynamic traffic networks.Considering the impact of traffic patterns and traffic costs on accessibility of medical services makes more sense in cities with obvious traffic congestion and more developed public transportation systems.Taking Shanghai as an example,this paper studies the accessibility of medical services in different travel modes and different modes of transportation based on the improved two-step floating catchment area method.This paper is devoted to analyzing the spatial layout of medical resources to provide recommendations for the allocation of medical resources.This article mainly completes the following work:(1)This paper obtained medical service facilities data,population data and transportation network data of Shanghai and established a geographic database.(2)This paper used map service to obtain the weighted optimal time path of walking,driving and public transportation trips in different time periods,and constructed the OD time cost matrix.(3)Based on the fine-grained cell population aggregation data,this paper considered the hospital's service capabilities and improved 2SFCA by introducing the gravity attenuation function.On this basis,this paper calculated the medical services accessibility under different search radii,different time periods,and different modes of transportation,and analyzed spatial distribution patterns and changes.(4)Based on the comprehensive utilization of location entropy,Lorenz curve and Gini coefficient,this paper analyzes the equality of the distribution level of medical facilities.According to the global correlation coefficient and the local correlation coefficient,this paper analyzes the spatial correlation and local correlation phenomenon of the accessibility of medical services.This article is dedicated to provide advice on improving the quality and uniformity of medical services in Shanghai.The main conclusions of the study are as follows:(1)The accessibility of medical services in Shanghai presents a continuous layered distribution.Core layer polarization is severe.Accessibility is attenuated from the central city to the periphery.Accessibility shows significant differentiation between the east and the west in the surrounding areas of the main city.The public transport accessibility core circle has a radial distribution,due to the influence of rail transit.(2)Medical service accessibility and search radius are related.Urban core area is more sensitive to changes in search radius.(3)The accessibility of medical services in Shanghai showed obvious characteristics of time division differentiation.The accessibility when the road is unblocked is better than the accessibility when the road is congested.The time sensitive areas are mainly distributed in the areas with the highest congestion index.(4)The allocation of medical resources in Shanghai is uneven.Shanghai's medical resources are too concentrated in the core of the city.The level of medical services in the suburbs is generally low.In summary,Shanghai needs to improve the allocation of medical resources and public transportation in the suburbs.
